Note: This Quarterly Progress Report must be submitted to EOPSS Office of Grants & Research along with the Quarterly Financial Report and Excel Spreadsheet within fifteen days after the end of the quarter. The information in this report must reflect all activities that were both federally and match funded (if applicable) for the VAWA STOP Grant Project.
I. Goals, Objectives and Activities
Explain your progress in reaching your project’s goals and objectives in this quarter. Fill in the chart below with your program’s Goals, Objectives, and Activities.
Under the Activities section please include any of the following:
· Direct services
· Personnel activities (i.e. hiring, promotions)
· Policy changes/enhancements (i.e. implementation of guidelines)
· Collaborative efforts (e.g., community based-events, commission meetings, advisory committees, and community roundtables) your agency has organized or participated in related to your VAWA funded project. Please include the participating agencies and the topics of discussion.
· Trainings, conferences or seminars that the personnel employed through your VAWA STOP Act grant attended. Please include the location and topic of each, as well as the VAWA staff that attended
When submitting reports for Quarters 2, 3, and 4 please keep all the activities/meeting/trainings/etc in the report from the previous periods. The example below shows a progress report for Quarter 2 (notice how the Quarter 1 activities are still in the report. This will help EOPSS and you gauge the success of your program and each goal. If no activity for a particular objective happened in a quarter, please indicate ‘Not Applicable.’ If more boxes are needed please add where appropriate.

(For categories that are not applicable, write “N/A.”). An agency is eligible for a category if they received VAWA funding for that specific category. Additionally, please attach a copy of all materials produced with VAWA STOP Act Grant funds in this quarter.
