AWARD OF MERIT – OT

PURPOSE:

To recognize an occupational therapist who has made a notable contribution to the profession in one or more specific areas of occupational therapy practice.

REQUIREMENTS:

OT PRACTITIONER
Must be licensed in New Jersey as an occupational therapy practitioner.
AOTA membership preferred.
NJOTA membership required.
Must have made a notable professional contribution towards practice such as through:
·  Development of treatment strategies, practice models, evaluation/assessment tools, or programs that enhance consumer service delivery.
·  Participation in research to benefit occupational therapy practice.
·  Participation in advocacy based activities towards the enhancement of the profession and benefit of its consumers
Must have a minimum of three years’ experience specializing in an area and recognized by peers as an expert in their field.
Nominator will submit a letter of recommendation, the practitioner’s CV, and the application for the award.
Submit 2 additional letters vouching for the practitioner’s outstanding leadership.

SELECTION:

The deadline for submitting the application with all supporting materials is
September 15, 2015.

Nominations shall be addressed to the NJOTA Awards Committee. The Awards Committee will review applications for completeness and appropriateness and make recommendations to the Executive Board for final approval.

PRESENTATION:

Award will be given at the annual NJOTA Conference.
