DO the Action Plan
What went wrong? What happened that was not part of the plan?
Sheffield – look at 4 basic tasks on shared care and start with hand washing – knew it was an issue with patients. Started off looking how many patients were washing hands – identified shifts to look at (bays/shifts) – showed quite poor in hand washing. Not intervened, just monitored – identified potential issues in each unit – not enough facilities in different units etc. identified problem that was hand washing, collected baseline data, showed problem existed, also identified physical layout of unit probably means single solution isn’t going to work. Engaging staff – main unit – volume of staff and time constraints mean less engagement.
STUDY
Complete analysis of data. Summarize what was learned include results of predictions.
Sheffield – Learning about how staff can alter patient’s behaviour more – the PDSA identified staff issues and showed shouldn’t presume that staff do/know what you do. How staff can alter pt behaviour more; not ‘hello how are you’ but hello have you washed your hands?’
PH-hypothesis about what to do to change behaviour; ‘go to where the energy is’. Who is keen?

What decisions were made from what was learned?
Sheffield – peer support, break the habit and change the habit, hello wash your hands
What will be the next cycle?
Sheffield – identify why hand washing not undertaken by patients = look at patients on same shift, ask if washed hands, if not why, and if they understand necessity to wash hands. Look at education, facilities, how to encourage patients to wash hands.
