WELCOME TO OUR PRACTICE
This leaflet is designed to welcome you to our practice, and make you more familiar with our staff and services run from our practice in the health centre.

Your views on the services offered and any ideas on changes or improvements are always welcome.

We make every effort to ensure that patients experience at the practice is as comfortable and professional as possible. If you have any complaints or suggestions, please write in confidence to the practice manager.

PATIENT REGISTRATION AND BOOKING APPOINTMENTS

NEW PATIENT REGISTRATION
Contact the practice on 020 8520 7115 between 9.00AM and 12:30PM, and between 2PM and 6PM, except Thursdays to book your health check appointment.

All new patients are given a health check with our health care assistant. This is the opportunity to obtain important medical history until your notes arrive from your previous GP.

The health check must be booked a month in advance of your application. Everyone 5 and over should have the new patient health check.

New patients who fulfil government criteria of eligibility to NHS care are able to register with this practice, after an application process, with this practice.

·  All new patients over the age of 5 years will be offered a health check appointment with the practice nurse.

·  Patients over the age of 75 are seen annually in the surgery or visited at home.

·  All patients over the age of 75 now have a named GP at L.L. Medical Care Ltd (Agarwal & Agrawal Practice). Patients can enquire about this at our reception.

You will also be asked to provide;

·  Your medical card (from GMS1 to be completed if this is not available).

·  Photographic proof of identification - driving license, ID card, birth certificate and proof of address (dated within 3 months).

·  If you are registering children under 10 years of age, you must provide copies of all immunisations.

If you have had any previous medication, or have had a change of medication, remember to bring copies of your green form, which is attached to every prescription, on request.

APPOINTMENTS

BOOKING APPOINTMENTS

·  Appointments are made by visiting personally or by telephoning the surgery.

·  Booked consultations take place within the practice.

·  Urgent cases will always be seen on the same day, otherwise you will be offered the first available appointment with a doctor/triage staff within 48 hours. However this maybe affected by unforeseen circumstances such as sickness and holiday leave.

·  Appointments will be 10 minutes long, but if you feel you may need longer please let us know.

·  Emergencies and babies will be seen on the same day.

·  Urgent telephone consultations are available between 12.00PM and 12:30PM only.

·  Extendable opening hours are available in the evening.

·  Occasionally you might be asked to complete a short 30 second questionnaire regarding your experience with the practice.

If you are unable to make your consultation, please contact the practice 24 hours before your appointment is due so that another patient can have that slot, and remember 1 person per appointment please, thank you.

BOOKING APPOINTMENTS ONLINE - NEW
We have now introduced a new Online Appointment Booking System. To use this new service, please collect a registration letter from 1 of our receptionists or download a registration letter from our Appointments Page on our website.

EMERGENCY CALLS

·  During practice hours, please telephone the practice and your message will be relayed to the doctor.

·  During out of hours the NHS on NHS 111.

EMERGENCY APPOINTMENTS

For weekend GP Health Services, please call the Highams Hill Medical Centre. This service is commissioned by the Waltham Forest Clinical Commissioning Group and is open on Saturdays and Sundays, from 9.00AM to 5.00PM. They can be reached by dialling 020 8519 3999.

You can also contact NHS 111 for urgent medical matters or advice.

Please think before you call. Please help the doctors and other patients by being considerate.

HOME VISITS
Please do not request a home visit for inconvenience reasons, for example, the lack of transport, as a refusal may offend, but is both medically and legally justifiable and acceptable.

·  If possible please try to telephone reception before 11:30am if you require a home visit.

·  You may only request a home visit if you are housebound or are too ill to visit the practice.

·  Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

·  Please be aware that it is not possible to specify which clinician will attend any emergency home visit as they are delegated to the clinician on-call at the time.

During practice hours, please telephone the practice and a Dr will return your call as soon as he/she can.

TEST RESULTS
Please call between 11:00AM and 12:00PM to enquire about your test results.

When you take your test you will be told how long roughly it will take the results to be ready.

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate to.

PRESCRIPTIONS

·  To avoid errors, please do not order prescriptions by phone.

·  Please allow 48 hours if collecting and 3 days if enclosing a stamped address envelope.

·  Patient's on repeat prescription systems should see the doctor at regular intervals as arranged by the doctor between 6-12 months for reviewing.

·  The counterfoil side of the prescription is the side given when collecting a prescription.

·  Most local pharmacies provide the services of pickup and delivery once authorised by patients are able to visit during it's opening hours.

·  Most local pharmacies will provide advice related to minor ailments such as coughs, colds and diarrhoea over the counter.

MINOR AILMENTS SCHEME
It is not always necessary to come to the surgery for minor problems, most pharmacists offer a “Minor Ailments Service”. You can use this service if you need things such paracetamol, certain creams and hay fever medication.

If you speak to your pharmacist they will be able to advise you if they can help you or not, or if you should see the doctor.

If you are entitled to free prescriptions, you will not be charged for this service.

BY BUS
Take the 357 bus from Stop KA, towards Chingford Hatch, get off at Jewel Road (Stop BA), walk ahead to the surgery.

BY CAR
Head south east on Station Approach, turn left on Hoe Street/A112 and follow the road, turning left at Forest Road.

BY FOOT
Head south east on Station Approach, turn left into Hoe Street, then turn left into Greenwood Road, then left into Forest Road.
