Student Application for Health Care and Medical Students

Dear Student:
The Dr. Mehrdad Shafa Medical Scholarship Foundation (MSMSF) was created through the generosity of public and private funding in Phoenix, Arizona in 2012 as a 501(c)(3) as a public charity. The scholarship was established to provide financial support for Pre-medical, Nursing and Allied Health Sciences for Arizona students.
Arizona State Law (A.R.S. 1-501, 1-502) requires that Dr. Mehrdad Shafa Medical Scholarship Foundation (MSF) in administering any federal public benefit or state or local public benefit must require that each natural person who applies for the benefit to submit documentation along with a signed affidavit stating that the documents are true.

The MSMSF Scholarship Application requirements:
- The student meets the State of Arizona and the United States of America lawful residence requirements.
- The student must be enrolled as a full time, residential, non-correspondence student with a major in the pre-medical, nursing or allied health science educational program in Arizona.
- The student demonstrates a grade point average of 3.0 or above.
- Applicants may be eligible for additional scholarships which are awarded annually based on merit and financial need.
- The student may be required to appear for a personal interview.
Scholarship award amount: $1,000.00 - $5000 Annual.

- The MSMSF Scholarship Application must include the following documents:
- (3) Three lettersof recommendation from a non-relative (teacher, counselor, professional, employer, mentor, clergy).

- A copy of applicant’s current, complete transcript. Transcripts must display student name, school name, grade and credit hours earned for each course and term taken.
- Applicant’s comprehensive essay.
- A 4 x 6 head shot Photograph.
If you are Selected you must provide the following before a final decision will be made:
- The Financial Award Letter.
- US citizens must submit proof of citizenship; Permanent Residents must include a copy of your
- Permanent Registration Card.
- Confirmation of enrollment and school schedule must be submitted to MSMSF before scholarship awards will bemade to your college financial aid office.
- You may be required to have your application notarized upon request of MSMSF.
The personal statement should describe your career goals and how the scholarship will assist you with achieving your goals. Please include any relevant information including past accomplishments, specific activity or experience that has been important in clarifying or strengthening your commitment in the medical/health sciences field. The essays are very important to the Selection Committee in making their final decisions and should be no longer than 500 words.
The completed application along with the required supporting documentation must be submitted or postmarked no later than March 30th annually for the following fall semester.
