The Dayton Emerging Fashion Incubator (DE-FI) Launch Party is taking place in the majestic setting ofSunWatch Indian Village, the DE-FI's Launch Party will be the hottest event of the summer and the place to seeand be seen! Attendees will have the opportunity to mix, mingle, and network with some of the area’s topprofessionals, community leaders, and fashion lovers from all over. Complete with an official introduction ofthe DE-FI team and Incubator designers...the night will also include one of the fiercest fashion shows aroundfeaturing local and nationally known designers who will be prepared to take special orders on all of the designs

that they will be showcasing. Then we will dance the night away under the moonlight giving all attendees a night atthe museum that they will never forget!! Be a witness...#FirstFlightNowFashion

If you are interested in participating in the “Fashion in the Mystic Garden” Launch party, please fill out thisapplication form and submit it with your payment. You will receive an email confirming your application andparticipation. Vendors will be given priority booth assignments based on the date of sign up and payment.

The “Early Bird” rates are in effect until June 13, 2013 – however placement of vendor space is based onthe date that (DE-Fi) receives the application AND payment. Space is limited and once all vendors’ spaceshave been filled, we will no longer accept applications for the Launch Party.

NOTE: (DE-FI) reserves the right to refuse vendors based on Launch Party criteria.

Launch Party Date: Sat., July 13, 2013 6:00pm-11:00pm

Set-up: Saturday afternoon beginning at 2 p.m., with final set up by 4 p.m. sharp.

●A. COMMERCIAL VENDOR FEE:

10 x 10 Space: FEE: $250 (until June 13, 2013). $300 (after June 13, 2013) Fee can also be offset by selling tickets to the Launch Party; General Admission tickets are $45 and VIP tickets are $65. Fee includes: 1 table & 2 chairs, listing and logo in (DE-FI) Fashion Guide, Brand/logo on website, on sitesignage, (2) Exhibitor passes.

10 x 20 Space: FEE: $350 (until June 13, 2013). $400 (after June 13, 2013) Fee can also be offset by selling tickets to the Launch Party; General Admission tickets are $45 and VIP tickets are $65.

Fee includes: 1 table & 2 chairs, listing and logo in (DE-FI) Fashion Guide, Brand/logo on website, on sitesignage, (2) Exhibitor passes.

●B. COMMUNITY SPACE/Dayton Departments

Fee: $50 Fee can also be offset by selling tickets to the Launch Party; General Admission tickets are $45 and VIP tickets are $65. Offered to organizations which have a non-profit 501 C3 status* and to City of Dayton Departments. Groups must provide their own booth canopies, tables and chairs. There is limited space offered in thiscategory. Please contact Jennifer Howard at .

C. NONPROFIT SPACE:

Fee: $100 (discounted)

Fee includes 10x10, 1 table & 2 chairs. Fee can also be offset by selling tickets to the Launch Party; General Admission tickets are $45 and VIP tickets are $65. We offer this space to organizationswhich have a nonprofit 501C3* status. Nonprofits must submit and be accepted for this space. There are a limited number of spacesoffered for this category.

●D. KIDS CRAFT AND DEMONSTRATIONS:

Fee: $50

The Dayton Emerging Fashion Incubator (DE-FI) offers a 10x10 with 1 table and 2 chairs toorganizations whichhave anonprofit 501 C3* status providing cultural demonstrations or free crafts

for kids without sales. This is offered to groups at the discretion of the (DE-FI). Fee can also be offset by selling tickets to the Launch Party; General Admission tickets are $45 and VIP tickets are $65. There is very limited spaceavailable. Please contact Jennifer Howard at .

*Note: IRS nonprofit status must be provided with the application form

Vendor Booth Application

●Health Permit Fees are not included [Food Vendors including sampling and packaged foods].

●All correspondence will be via email unless special arrangements are made. If a vendor does not haveemail, the vendor must check the (DE-FI) website for updates.

●This is a 1-day Launch Party. Vendors must utilize their spots and remain at their space during the

Launch Party Hours or it may affect your participation in future festivals.

●Vendors cannot shut down prior to the close of the Launch Party for any reason without prior writtenauthorization from the Festival.

●Commercial and Food Vendors - Provide a copy of your Insurance Certificates of Liability naming theLaunch Party on the Rider, under the name Dayton Emerging Fashion Incubator (DE-FI).

●If you are selling, please provide a copy of your Ohio Vendor’ Permit certificate issued by the

State,City, and/or County. Vendor’s Permit will be checked before booth setup.

●A Cleaning/Security Deposit of $50.00 (to be held, but not cashed) for all vendors and $250.00 for

food vendors will be required by August 15, 2011. Checks will not be cash and returned at the close ofthe Festival if the booth is left in an acceptable manner as determined by Festival organizers. If

Vendors do not check out at the end of their festival – they forfeit their deposit. Check out procedures

will be provided.

●The Launch Party is held outdoors and subject to nature. There are no refunds for rainy or unusual

weather conditions or acts of God.

●There are no refunds once you have been accepted into the (DE-FI) Launch Party.

●The (DE-FI) Launch Party has the right to determine participation – once you have submitted your

application you will receive notification of acceptance/rejection.

●Vendors must be paid in full by June 30, 2013 in order to participate in the (DE-FI) Launch party.

“Key area” placement of booth space is based on payment date of vendor fees.

●No sound systems are allowed in the vendor space without prior permission from (DE-FI).

●A Vendor Agreement will be emailed to you by July 1, 2013 outlining responsibilities of the Vendor.It will include: check-in time, parking information, booth location, and other Launch Party information.

●The Vendor Agreement Form must be signed, dated and received by July 8 , 2013 along with your

Cleaning/Security Deposit.

●Volunteers are available for unloading/loading vehicles to help with traffic flow. Volunteers are not

available to pack or set up/tear down booths. Volunteers are not allowed to work in Vendor Booths.

●Vendors deemed unmanageable during (DE-FI) will be given one warning by the Launch Party

organizers. (DE-FI) reserves the right to remove any vendor who is deemed unmanageable by LaunchParty organizers. There will be no refunds to vendors who are removed.

●Note: Due to the nature of the Launch Party some items on this document may change as to set up

time, locations, etc. If it does, Vendors will be immediately notified.
