Job Title : Mental Health Service Coordinator , SHA Scattered Sites

Job Summary : Provide information,and referral for a wide array of services
to clients residing in Seattle Housing Authority (SHA) Scattered
Sites communities.Services provided range from support with behavioral health and/or chemical dependency issues, accessing primary medical care, accessing rental assistance resources, enrolling with a representative payee, and problem-solving issues which may negatively impact clients’ ability to maintain their housing.Work as an advocate for clients, in order to stabilize their housing status and prevent eviction. Must adhere to Full Life Care, State and County policies and regulations.
Reports to : Full life Care SHA-Outreach Program Manager.
FLSA Status: Non-exempt
ESSENTIAL FUNCTIONS of the job:
- Maintain quality client services by following standards and policies and
implement program plans.
- Receive and respond to referrals made by Seattle Housing Authority (SHA)
Property management staff and others, to provide services to elderly and non-elderly disabled clients.
3. Work collaboratively with SHA residents, conduct on-site client needs
assessments, and develop mental health case management plan, to access
needed resources.
4. Complete regular outreaches to individuals and families residing within SHA’s Scattered Sites pulic housing portfolio, throughout Kind county. Must have reliable transportation, in order to complete outreach work and to offer services.
5. Determine if residents are currently enrolled with a mental health provider
and assist them to either re-connect or to enrollin services when needed, to
support their behavioral health needs.
6. Provide advocacy, translation and direct assistance to low-income clients, to
enable them to access needed resources.
7. Monitor client progress, to determine if the outcome of referrals and
stabilization services was successful. Provide services at a level of not less
than 40 hours per week.
8. Attend weekly team meetings, and participate in regular case consultations
with colleagues and Program Manager.
9. Establish a compassionate and respectful environment by practicing good
communication skills; maintain client confidentiality, conduct self in a
professional manner.
10. Maintain a safe and healthy environment, adhere to infection control
protocols; address potential safety and health hazards; follow emergency
policies and procedures; fulfill education and certification requirements.
11. Maintain required documentation to meet WAC and HUD guidelines, as
well as Full Life Care policy.
12. Complete projects and other tasks as assigned by Program Manager.
Educational Requirements Needed to Perform the Duties of the Job:
- Bachelor’s Degree in related field i.e. Psychology, Sociology or Mental Health
studies as well as a minimum of one year of related professional working
experience..
- Must be certified in CPR and attend HIV/AIDS information training.
Physical Requirements Needed to Perform the Duties of the Job:
- Must be able to lift 25 lbs. and support clients’ mobility.
