Please complete questionnaire completely and thoughtfully. This process is extremely important for several reasons:

·  The process helps you think about yourself - your past, present and future; your strengths and weaknesses; your goals and aspirations – as you begin your college search.

·  Teachers and counselors will use this information to write your college letter of recommendation. The more information that is shared, the better prepared these professionals will be to write letters that most reflect you as a whole person.

Deadline: By the first day of school next year. You can turn into me before then or email it to me over the summer, but I must have a copy on the first day of school next year. You cannot schedule your senior conference unless I have a copy of your Senior Questionnaire in hand!

Feel free to attach a resume or any other extras that may better help inform us of all your involvements.
