- Procedures
- Secure a Request for Changes or Reconstruction from the parish secretary duly signed by the parish priest and sealed with parish seal.
- If the petitioner cannot personally perform the procedure, he/she must have a valid reason for not being able to come personally and must write an Authorization Letter. A valid id of the petitioner and of the representative is required.
- The parish secretary must inform the petitioner about the required documents that would accompany the request. Note for parish secretaries: Please include the Request the date of baptism. Please inform also the petitioner that the request is not yet final and is subject to the approval of the Office of the Notary on Canonical Books.
- The petitioner, after obtaining the required documents, proceeds to the Office of the Notary for evaluation and processing of the request and consequent approval if request is found meritorious.
- The Office of the Notary issues a Decree of Authorization for the parish priest to make the approved changes in the sacramental registers. The Decree must be signed by the Notary and sealed.
- The petitioner returns to the parish secretary and present the Decree.
- The parish priest/ secretary issues corrected Baptismal Certificate. Note for Parish Priest: Please do not make corrections on the entries by erasing or adding data recorded on the books of Baptism and Confirmation. Instead, the necessary notation should be made on the space provided for in the OBSERVANDA: Date Corrected: Changes Made: See Decree. The Decree must not stapled to the page of Bok, but rather be kept in a separate parish file.

- Misspelling of Child’s Name
Certified True Copy/ Authenticated Copy of Birth Certificate newly issued by the National Statistics Office (NSO) OR THE Local Civil Registry Office (LCRO)
Two (2) valid IDs (school, SSS, Postal ID, Voter’s ID)
Form 137 (Elementary, High School) or Transcript Records
Graduation Diploma/Certificates
- Parents Name
Certified True Copy/ Authenticated Copy of Birth Certificate newly issued by the National Statistics Office (NSO) OR THE Local Civil Registry Office (LCRO) of the Child.
Certified True Copy/ Authenticated Copy of Birth Certificate newly issued by the National Statistics Office (NSO) OR THE Local Civil Registry Office (LCRO) of Parent Concerned
Marriage Contract of Parents
Two (2) valid IDs of the parent concerned.
- Place of Birth
Certified True Copy/ Authenticated Copy of Birth Certificate newly issued by the National Statistics Office (NSO) OR THE Local Civil Registry Office (LCRO)
Certified True Copy of Hospital Record for Children born in hospital.
Certification from the Local Civil Registrar of the child’s place of birth.
- Date of Birth
Certified True Copy/ Authenticated Copy of Birth Certificate newly issued by the National Statistics Office (NSO) OR THE Local Civil Registry Office (LCRO)
Late registered Birth Certificates will not accepted
Form 137 (Elementary, High School) or Transcript of Records in College with date of Birth.
Two (2) valid IDs with Date of Birth.
- Adoption
Certified rue Copy of the Court’s Decision on the Adoption
Adoptive Parents marriage Contract
Certified True Copy of Finality of Decision
Un- amended and Amended Birth Certificates from NSO or LCRO
Note for Annotations:
- After Baptism:
- Full Name of Adopting parents
- Number/ date of the Decree of Adoption issued by the civil court.
Before Baptism
- If adopting parents request that names of the natural parents be kept confidential, in accordance with civil laws:
- Do not enter full name of natural parents
- But, forwarded to the Secret Archives of the Chancery
- Released only upon approval of the Bishop and for serious reason.
- Annotated in the Baptismal Register: “For Marriage purposes, see Secret Records of Diocesan Archives”
- If not, follow usual entry
- Non- Permissible Changes

Date of Birth if Birth Certificate is Late Registered
Change of Parish Church where the wedding took place
- For Cases Without Birth Certificate
Certification of No Record from NSO and LCRO
Marriage Contract of Parents

Form 137 (Elementary, High School) or Transcript of Records in College with date of birth
Graduation Diploma / Certificate
Certified True Copy of Hospital Records for children born in hospitals
Notarized Joint Affidavit of Two (2) Disinterested Persons (Witnesses)
- For Legitimation by Subsequent Marriage/Inclusion
Un-amended and amended Birth Certificate
Marriage Certificate of Parents
Certification on Acknowledgement of Paternity
Upon approval, the Name of the Father can be written in the column provided for in the Register, and the surname of the child will carry the father’s surname.
- For Full Reconstruction of Records
Certified True Copy / Authenticated Copy of Birth Certificates newly issued by the National Statistics Office (NSO) or the Local Civil Registry Office (LCRO) of the Child
Parent’s marriage Contract
Form 137 or Certification from the School
Picture taken during the ceremony
Invitation
Memorandum of Baptism or Receipt of Baptism
Confirmation Certificate (For baptismal Reconstruction)
Note: THIS IS A MUST FOR PARISH SECRETARIES: You are to advise the applicants to schedule an interview with the parish priest, especially for those who will not be able to produce the required documents/ evidence that the ceremony has taken place.
NOTA BENE: REQUEST FOR REMOVAL OF FATHER’S NAME/FAMILY NAME WILL NOT BE HONORED.
