6. BRIEF RESUME OF THE INTENDED WORK
6.1 NEED FOR THE STUDY:
Shalya Tantra is a branch of Astanga Ayurveda. Siragata vyadhi is characterized by Shoola (Pain), Aakunchana (Constriction) and Purana (Engorgement) 1, 2. All these symptoms can be co-related to the symptoms of Superficial Thrombophlebitis.
It is a common disorder of veins, affecting a significant percent of population irrespective of sex and race. Superficial Thrombophlebitis is a frequent complication of varicose veins and deep vein thrombosis. This may also follow cannulation of a vein for an intra venous infusion3 .According to contemporary science the line of treatment is – Elevation, anti inflammatory drugs, antibiotics, application of crepe bandage and Heparin Gel, even then reoccurrence and chances of complications are more.2So there is a need to find a safe and effective therapy.
In classics we get various references regarding the use of Raktamokshana in Siragatavyadhi.4, 5, 6. Jalauka is considered one of the best modality of Sthanika Raktamokshana.Hence this study is planned to evaluate the effect of Jalaukavacharanain the Management of Siragata Vyadhi.
6.2 Review of the literature:
1) The common and specific Nidana of Siragata vyadhi are explained in text asSamanya Nidana ofVata vyadhi.7, 8, 9, and 10.
2) The Samprapti of Siragata vyadhi are explained as Samanya Samprapti of Vata vyadhi.11
3) The Lakshanas like Shoola, Aakunchana, Purana, etc are explained as the clinical features of the disease Siragata vyadhi by all the Acharyas.12, 13.
4) Raktamokshana is explained as the treatment principle of Siragata vyadhi.14
5) Clinical features of Superficial Thrombophlebitis according to Contemporary Science like Pain, Constriction, Engorgement of Vein.15
6) Research conducted on Superficial Thrombophlebitisat differentResearch centers, relevant journals, periodicals, magazines will be reviewed.

6.3OBJECTIVES OF THE STUDY
- To evaluate the efficacy of Jalaukavacharana in Superficial Thromboplebitis.
- To standardize the number of sitting and duration of Jalaukavacharana in Superficial Thromboplebitis.
7.MATERIALS AND METHODS
7.1SOURCE OF DATA:
Patients will be selected from out patient department and in patient department of P.G.Studies in Shalya Tantra, S.D.M.college ofAyurveda and Hospital, Hassan.
7.2METHODS OF COLLECTION OF DATA:
Patients will be registered with the help of research proforma prepared for the study.
DIAGNOSTIC CRITERIA:
Diagnosis will be made on the basis of Lakshanas of Superficial Thrombophlebitis.
INCLUSION CRITERIA:
Patients having clinical features ofSuperficial Thrombophlebitis like Pain, Constriction, and Engorgement of Vein.
EXCLUSION CRITERIA:
- Embolism.
- Bleeding disorder.
- Diabetes Mellitus.
- H I V and HbsAg.
- Infection Cases.
TREATMENT GROUP:
30 patients fulfilling the criteria for inclusion will berandomly divided into the following two groups.
Group A:The patients of this group will be treated byJalauka on lesion,alternate days for 7 daysup to4 sittings.
Group B:The patients of this group will be applied Heparin Gel on lesion, alternate days
for 7 days up to4 sittings.
Procedure:
The patient is made to sit or lie on the bed. Jaulaka is to be applied on the site. Jaulaka will bite on the skin and suck the blood itself. Its face appears like the hoof of a horse and it raises its neck by this we can understand that it has started sucking blood. As soon as Jaulaka starts sucking, wet white gauze will be covered on it. After sucking ashuddha Rakta patient gets itching and pain at the site, then it is removed by sprinkling Haridra churna at its mouth region.
7.3 LABORATORY INVESTIGATION:
- HB%.
- Total Count
- Differential Count.
- Erythrocyte Sedimentation Rate.
- Clotting Time.
- Bleeding Time.
- Fasting Blood Sugar.
ASSESSMENT CRITERIA:
1. Assessment is done on the basis of improvement in signs and symptoms.
2. Patients will be assessed with subjective and objective parameters formulated for Superficial Thrombophlebitisbefore and after the treatment.
SUBJECTIVE PARAMETERS:16, 17
- Shoola (Pain).
- Akunchana (Constriction).
- Purana (Engorgement).
OBJECTIVE PARAMETERS:
- Discolouration.
- Redness.
- Tenderness.
- Thickness of Vein.
DURATION OF TREATAMENT:
Duration of Jalaukavacharana will be carried out for maximum 7 days.
FOLLOW UP STUDY:
After completion of the treatment the patients will be followed at regular intervals of every 7 days for a period of 2 months.
