TIPS FOR COMPLETING SCIENCE FAIR PAPERWORK 2017-18

Required
1: Checklist for Adult Sponsor: This is signed by the person who supervises your project.
- Project Title on this page will match every other page and your board.
- Be sure to check any applicable boxes.
1A: Student checklist: Complete entirely.
- Actual Start Date is the date that you began the actual experiment. BE SURE THAT THIS DATE IS THE SAME AS OR LATER THAN THE DATES FOR PERMISSION.
- Your end date CAN be after your school fair date.
1B: Approval: You and your parent sign and date.
- THE DATE ON THIS MUST BE THE SAME AS OR LATER THAN YOUR EXPERIMENT START DATE.
- If a team project (up to 3 members) each completes this form.
- Human projects or Potentially Hazardous Biological Agent projects (microorganisms, rDNA, etc) require approval from your School Scientific Review Committee
Research Plan: required with above paperwork in order to review your plan. Include:
- Rationale/research
- Hypothesis or expected outcome/research goal
- Describe as best as you can your procedures including methods for data collection
- Your plan may morph or change; that can be explained in research report
- Risk and Safety: Identify within the plan and precautions needed
- If personal protective equipment required, this should be listed as first step of your procedure
- For disposal of materials, you must fully list procedure including length of time and temperature for autoclaving, etc.
At end of project, addend your “plan” with a report, noting in detail any changes. You must have data analysis and a conclusion and at least five bibliographical resources cited.
In addition, at the end of your project, you will need to give your teacher:
- The current State Science and Engineering Fair Form signed in blue ink
- An abstract of no more than 250 words
If you work in a Biosafety Level 2 Laboratory, you will require:
- A BSL 2 Checklist
- A Qualified Scientist Form (2)
If you work with microorganisms or cell lines, you will require:
- Potentially Hazardous Biological Risk Assessment Form (6A)
If you work with cell lines or tissues, you will require:
- Human/Vertebrate Animal Tissue Form (6B)PonteVedra High School is NOT a Regulated Research Institution. Students who work in an industrial setting or outside research area will require:
- Regulated Research Institutional/Industrial Setting Form (1C)
