JOB DESCRIPTION –Patient Navigator

DEPARTMENT: Client Services

REPORTS TO: Senior Program Manager

SALARY AND BENEFITS

  • Current FTE: 1.0 (40hours/week)
  • Starting pay $15/hr
  • Paid holidays, vacation time, and discretionary leave prorated to FTE

  • This position is *hourly/salaried and *exempt/non-exempt, i.e. *not/subject to state and federal wage and overtime requirements

THE POSITION

The Patient Navigator position will assist persons living with HIV/AIDS in Jackson, Josephine and Douglas counties who are experiencing multiple barriers accessing and retaining HIV care and treatment by providing Patient Navigator Services. The Patient Navigator will work to complement services provided by the clients Care Coordination Team by helping clients complete required paperwork, increase self-management skills, provide transportation assistance to medical and housing related appointments, accompaniment to medical appointments, and access medical and support services, including behavioral health care. The Patient Navigator will accept referrals from the Care Coordination team and work together with the client and team to develop a work plan.

RESPONSIBILITIES

CLIENT CARE

  • Client outreach and engagement
  • Provide direct one-on-one and phone navigation services
  • Make appropriate referrals, including follow up and documentation.
  • Collaborate with the SPM, RN and CC in the development of a work plan
  • Appropriately advocate for clients needs
  • Monitoring of service delivery
  • Communicate with SPM, CC and RN on work plan progress, client barriers and needs
  • Assist clients in getting to and from medical and behavioral health and addictions related appointments, to include picking up medications.
  • Provide health education, information and referral.
  • Assist the client in navigating through the health care system
  • Improve clients health literacy through provision of accurate information and skill building
  • Support the client in developing communication skills and strategies necessary to ensure productive communication with providers
  • Prevent and/or address identified barriers to care
  • Attend medical appointments for social support
  • Assist clients in completing paperwork for insurance or other benefits.
  • Assist the client in increasing self-sufficiency and self-management in addressing medical and behavioral service needs.

SUPPORT

  • Maintain knowledge of up to date referrals for clients in our service area
  • Develop and strengthen relationships with service providers in our service areas
  • Maintain up to date information regarding OR Medical Assistance programs
  • Maintain up-to-date HIV knowledge;
  • Maintain up to date information regarding other benefits and entitlements programs available in Oregon.
  • Maintain up-to-date and accurate client records, including CareWare
  • Participate in required training
  • Participate in necessary meetings
  • Assist Manager/Director with all necessary reports
  • Participate in agency teams
  • Other duties as defined by Manager/Director and Ryan White Principles and Standards

REQUIRED QUALIFICATIONS

  • Ability to set and maintain clear boundaries with vulnerable population
  • Excellent oral communication skills
  • Ability to work with people who are experiencing mental health or substance abuse issues
  • Detail oriented with excellent organizational and record keeping skills
  • Self-manager/self-motivated
  • Ability to work with diverse populations
  • Knowledge of local and statewide social service providers
  • Client-centered service experience
  • Willing to be part agency & department teams
  • Valid Oregon drivers license, driving record sufficient to be covered by agency auto insurance policy, ability to transport self to job related events
  • Ability to transport clients
  • TB test
  • Flexible hours required, some evenings and weekends
  • Ability to pass agency criminal background check
