Now, you're welcome to find another

The County Medical Society,
Can provide you with some leads.
We will continue to see you,
For thirty days and no more
But only for an emergency,
Shall you cross and enter our door.
We will forward your records as requested
To the doctor of your choice
But warn others of your debts and actions,
We shall not do by letter or voice
So this letter is our notice to you
Sent by certified mail at a pittance
In parting, we wish to express..
