Coronal Access #330,#1158 burs and #8 Round slow speed.
Locate canals with DG16 endodontic explorer.
#6 Endo Magic (25 tip 04 taper) then #2,3,4 Gates Glidden
Apex locator measurement using a #10 file with RC prep in canal.
#15,20,25, (25 tip) 03 taper Endo Magic files with RC prep in canal.
Repeat Gates #2,3,4 and wash with water.
25 tip 04 taper Endo Magic (EM) files used passively.
Irrigate with 6% NaOCl, then #20,25 EM files (to length)
(25 tip) 03,04 EM taper files
#35,#40, Endo Magic files to length with 6% NaOCl.
Develop apical seat with steel hand files.
Simplified obturation
Select a numbered gutta percha (GP) cone to the last file that fit passively in the canal (i.e.. #35).
Coat GP cones with Roth 811 sealer and place in canals.
Verify GP length with an x-ray.
Sear off cones at the orifice with a hot instrument (Touch and Heat).
Place a 21-gauge obtura needle into orifice and inject pushing needle out of the canal.
Place a 04 taper #25 or 45 condenser in a slow speed handpiece, run it full speed and place it into each canal. Go in and out of the canal 4 to 6 times and slide it off the wall.
Remove excess gutta percha from the orifice and pack the canals with a #5 endo plugger.
Dr. Chris Smith Endodontic Technique
-Microscope used throughout the entire treatment
-Anesthetic: Upper teeth: 1 carp Articaine, Lower teeth: 2 carps Lidocaine for mandibular block, 1 carp Articaine for buccal infiltration. PDL injections for teeth that are hard to get numb, X-tip or Stabident for teeth that still won’t get numb (very rarely have to use this).
-Isolate with rubber dam
-Access- round diamond bur (through crown), trans-metal bur, surgical length round bur
- find canals with microscope
- Open canals with SX protaper, then G.G. 2, 3,4. If canal is calcified, use small HF (6 or 8) to access canal and do some coronal flaring by hand
He will use the Endo Magic #15 and #20 files in tight canals and to get to the endo of calcified canals.
- Irrigate down each canal with 6% NaOCl
- get lengths with apex locator and usually a 10 file with RC Prep (canal reads better when dry)
- Open each canal to a 20 hand file
-Irrigate and keep canals and chamber full of NaOCl during cleaning and shaping with Protapers. Irrigate between each rotary file. S1, S2, F1, F2, Large canals F3 and profile 40/.04 taper. Sometimes use profile 50/.04 in larger palatal or distal canals. Sometimes I use a 35 Vortex file after the F2 and finish with that if the canal is too small or curvy for the F3
-In cases with narrow roots or in extremely curvy or calcified canals, I use EndoMagic files instead of ProTapers.
-Necrotic cases- EDTA 1 minute then final NaOCl rinse, Retx cases- MTAD 5 minutes
-Endoactivator used with EDTA and/or NaOCl.
-Dry with paper pts.
-Cones precut by assistants. Soak cones in NaOCl. Medium cones cut to size of last file in canal. F2-25 medium cone, F3- 30 medium cone, profile 40-40 medium cone, 35 vortex-35 fine medium cone.
-AH Plus/ ThermaSeal sealer
-Cone Shot
-Obturate with System B and Obtura (takes about 2-3 minutes)
-Cotton pellet with Alcohol to clean off sealer from chamber
-Cotton pellet and temporary or no cotton and permanent restoration.
