If you would like to put your child’s name forward for a club, please complete and return one of the attached slips. This slip will be your permission for your child to attend the club. You will only hear from the teacher again if the club becomes oversubscribed. If you hear nothing, please assume your child has a place at the club. Places will be allocated on a “first come, first served” basis. If your child cannot attend on a particular week please inform the school office or the teacher leading the club on the morning of the club. Please carefully make a note of the dates when your child’s chosen club is taking place as these do differ.

Please return your slips to the school office by Monday 14th September 2015 at the very latest.
