FINGERPRINTING

ACKNOWLEDGEMENT FORM

Central Washington University requires that all teacher candidates complete a fingerprint check and be cleared prior to being conditionally admitted to the Teacher Preparation Program and prior to pre autumn, practicum, traffic safety driving instruction, and/or student teaching. Please read the following important information carefully!

I understand that:

· I must be cleared by the WSP and FBI before I can be at least conditionally admitted and before I begin a practicum, traffic safety driving instruction, pre-autumn field experience, or apply for student teaching;

· I need to submit fingerprints prior to any field experience which requires clearance;

· Blurred prints or other circumstances may delay the clearance process;

· The fingerprint background check is valid for two years from the FBI clear date;

· All "yes" answers on the Character and Fitness Supplement must be cleared by SPI's Office of Professional Practices before I can begin a practicum, pre autumn, traffic safety driving instruction, or apply for student teaching; clearance on "yes" answers may take longer than the six month minimum stated above;

· Any falsification or deliberate misrepresentation, including omission of a material fact, in completing the Character and Fitness Supplement can be grounds for denial of certification;

· I must have a 3.0 grade point average for the last 45 graded quarter credits for full admittance into the Teacher Preparation Program, to student teach, and be certified;

· I must submit my graduation/certification application packet at the same time I turn in my student teaching application;

· I must show proof that I have purchased the $1 million liability insurance at the same time I apply for student teaching (unless applying for Fall quarter student teaching);

· I must have 75% of my major and minor completed before student teaching;

· No grades lower than a C will be accepted in my major, minor, or Professional Ed sequence;
