2011-2012Costume Order Form***IMPORTANT****PLEASE READ A.S.A.P.

A Non-refundable Costume Deposit of $45.00 per costume (per class) will be due by Saturday, November 19th, 2011, if you are planning on participating in the recitals in June, 2012. All students enrolled at “Julie’s” including new combination students are invited to participate in this year’s recitals to be held in June at the Performing Arts Center in Appleton. The date of the recital will be June16th and the dress rehearsal is scheduled for June 3rd, the time will be announced at a later date.

Note: If you are in more than one class, you must put a costume deposit down for each class you will be participating in. (example: Juv. B Corps; jazz and baton)

**If you are worried about your child’s growth, add an inch (or two) to the girth measurement.

Deadline for costume deposit is: November 19th, 2011

Any costume deposits received after this deadline will be assessed a $15.00 late fee and will not be guaranteed shipment for picture day. Please return the form below and amount due in an envelope with the student’s name and class day and time written on the front.

The average cost per costume in 2010-2011 was $48.00 for beginning combination students, $45.00-$55.00 for beginning level and child size costumes, $55.00-$65.00 for intermediate level classes, and $65.00-$100.00 for advanced level classes. We will be staying as close to these prices in ordering the best quality, styles, and fitting costumes for 2011-2012. ** Note: Some costumes run smaller than others, please measure your child carefully and add to those measurements if necessary. Costumes that are too small are extremely hard to fix and often it is too late to exchange them in time for pictures. Balances will be paid when costumes are in. All tuition balances must be paid in full in order to receive your costumes. (Exception: extra large adult and extra large child sizes are generally $10.00-$15.00 higher in cost).
