JOB SUMMARY: Provides self management education to patients with diabetes (Type 1, Type 2, gestational, and pre-diabetes) and their families. Provides ongoing education on evidence based treatment for diabetes for clinical staff.
EDUCATION, CREDENTIALS AND TRAINING REQUIRED: Graduate of an accredited and approved school of nursing. Bachelor of Science in Nursing preferred. Current licensure as a RN in the State of North Carolina or an unrestricted compact state license. Certification by the National Certification Board of Diabetes Educators at hire strongly preferred.
EXPERIENCE REQUIRED: Three years experience as a RN with diabetes self-management skills training experience preferred.
REPORTING RELATIONSHIPS: Supervises: None Reports To: VP Patient Care Services/CNO
WORKING ENVIRONMENT: Work will occur on clinical nursing units, educational classrooms, and in an office setting.
WORK AIDS: All related medical equipment, office equipment, audiovisual equipment and supplies. This list is not all-inclusive.
Essential Functions
· Demonstrates knowledge of diabetes management and education principles as appropriate for the age of the population served.
· Serves as a diabetes resource in the organization as well as the community.
· Assesses, plans, implements and evaluates effectiveness of individualized diabetes education services.
· Documents patient encounters and communicates encounters and recommendations to the physician.
