Instructions for Special Sworn Packet

Applying for Special Sworn Status

There are two phases of the process for obtaining Special Sworn Status:

  1. Submission of paper forms. All items listed below must be complete & given to your coordinator to be reviewed and to initiate the E-QIP process.
  1. Submission of the SF-85P via the Office of Personnel Management website. You will be invited via email to submit this form. Electronic copies are provided by your coordinator only so you can prepare to fill these out online.

After your coordinator gets all the forms from part one (see details below), he/she will FedEx them to CES. When they receive this information, they will send the Initial Information Sheet to the Office of Security in order for them to issue an invitation to complete the on-line form through E-QIP (Electronic Questionnaires for Investigations Processing). It is still a good idea to fill in the SF-85P based on the SF-85P tips sheet in advance as a worksheet, so you will know beforehand what information is needed when filling out the form on-line.

You have to complete the SF-85P on-line within 2 weeks of the invite e-mail from the Office of Security. Upon completion, you will be asked to print out 3 pages, sign them, and return to them to Security within 3 business days. You will also be asked to notify the Office of Security via email that you have completed the on-line form and have sent them the 3 signature pages. From the Office of Security:

"IT IS IMPERATIVE THAT SIGNATURE PAGES WITH ORIGINAL SIGNATURES BE SUBMITTED TO THE SERVICING SECURITY OFFICE NO LATER THAN THREE (3) WORKING DAYS AFTER YOU HAVE NOTIFIED THE SECURITY OFFICE OF THE RELEASE OF THE APPLICATION."

In addition to sending the 3 signature pages to the Office of Security, please fax these to Ann Schatzer at 301-763-5935. This is extremely important for us to ensure that your packet is complete.

It is strongly recommended that you keep photocopies or scans of all your completed forms.

Email your coordinator with questions on any of the forms.

PART 1:

Give paper copies of all of these items to your coordinator:

1.PIV Training Certificate indicating completion of personal identity verification (PIV) training. All applicants must go through personal identity verification (PIV) and PIV training. See the HSPD12 website: Training.html

a.Read through the information.

b.At the end, click on the link to receive credit for the training. Fill in the information about yourself (only fill in the required items).

c.Print certificate and give to your coordinator

2.Form I-9:

a.Fill in Section 1, remember to sign and data it.

b.Include legible copies of two IDs - one must be a Federal or State issued photo ID (one ID each from Columns A and B, or Columns B and C, which isdifferentfrom what is stated on the I-9 Form. Examples of acceptable combinations: A passport and a Driver’s license or a passport and a University ID with a photo).

c.You must send in 2 ID’s

d.RDC administrator must certify the ID’s and fill out that portion

e.If you are not a US citizen, please submit a copy of a permanent resident card or an unexpired foreign passport with an unexpired Arrival-Departure Record, Form I-94. The Alien # or Admission # is the number from the I-94 or from the permanent resident card.

3.BC-1759Special Sworn Status Form :

a.Fill out part a (but no need to fill in #7).

b.Print and sign the last page.

6.Initial Information Sheet

7.OF-306 Declaration for Federal Employment:

a.Fill out both pages and print it. Please type, rather than hand write, if at all possible.

b.Sign 17a.

8.Fingerprint cards

a.Cards are specific to the Census Bureau – get 2 cards from your coordinator.

b.You must have 2 completed cards.

c.Fill out all your personal information at the top of the card (e.g., name, height, hair color, etc). (You can skip “ORI”.)

d.Make sure the person taking the fingerprints signs and dates each card where it says “Signature of official taking fingerprints”.

e.You must also sign each card.

9.Data Stewardship Training (Note that User Name and Password are case sensitive)

Internet Instructions:

  • Complete the Knowledge Check questions.
  • Click the Get Your Completion Certificate link. After printing your completion certificate, close the course.

To obtain credit for completing this training, please follow the instructions provided in your training notification.

10.Title 26 Awareness Training (Note that User Name and Password are case sensitive)

Internet Instructions:

4. Read each screen of the course.

5. Complete the Knowledge Check questions.

6. Click the Get Your Completion Certificate link.

7. Please put your full name, phone number, and COTR name on your completion certificate.

8. After printing your completion certificate, close the course.

Please note:

·The User Name and Password are case sensitive

·A score of 50% or higher is required to receive credit for completing this training. 10 multiple choice and true/false questions display randomly in the Final Knowledge Check. For each question, select the best possible answer. The Knowledge Check may be retaken repeatedly as necessary. If you attempt the quiz at least 3 times and you continue to experience difficulty obtaining a score of 50% or higher, talk with your supervisor.

  1. One passport-quality photo. It must be on a white/light background. If you take it yourself, please email a JPEG file to your coordinator.
  1. If you are not a US Citizen, also include copies of your visa, your green card (if a permanent resident), and the picture page of your passport. If you are in the US on a student visa, please include a copy of Form I-20.
  1. If you are not a US Citizen, please fill out the Foreign National Visitor Info Form . Fill it out through Permanent Resident Number.
  1. If you are not a US Citizen, please fill out and sign DAO Attachment 3.

PART 2:

Preparation for online forms:

Your RDCA will also email you the PDFs of the SF-85P. You do not submit these to your coordinator. You will fill out online versions of them, when the Office of Security invites you to do so. However, the SF-85P requires a lot of information (some of which you might not recall off the top of your head), so it is a good idea to fill out the PDF ahead of time, so you know exactly what information they will ask you for. This is also a good idea, since the invitation to fill it out online is time sensitive. You have 2 weeks to fill it out, once you get the email inviting you to fill it out. Remember also, that you have 3 days once you complete the online form, to FedEx in the three signature pages.
