2018 Instructions for Completing the New Application Form for Participation in the
Commonwealth of Massachusetts Employees Charitable Campaign (COMECC)
Applications shall be submitted to the Statewide Campaign Manager conducting the Commonwealth of Massachusetts Employees Charitable Campaign.

Region: Region 1: Essex, Middlesex, Norfolk, and Suffolk Counties
Region 2: Barnstable, Bristol, Dukes, Nantucket and Plymouth Counties
Region 3: Worcester County
Region 4: Franklin, Hampden and Hampshire Counties
Region 5: Berkshire County

If you are a Federation affiliate, send your application and electronic 25 word description file (xls) directly and only to your Federation!
For participation in the Commonwealth of Massachusetts Employees Charitable Campaign, copies of the following information, pertinent to your Unaffiliated organization or Federation or Federation affiliate, must be included with your New Application (available at www.comecc.net). If a federation is applying, each organization within the Federation must supply full documentation. (See numbers 1 through 9 of application).
1. A photocopy of Certificate for Solicitation (from MA Attorney General’s office) with an end date of
May 31, 2016 or after. To receive/renew your certificate of solicitation you must complete Schedule A-1 and Schedule A-2 of the Form PC and submit it to the Division along with the Form PC and appropriate filing fee. 2. Copy of IRS 501 (C) (3), Letter of Determination. The IRS sends you this letter after you have successfully applied (IRS Form 1023) for the recognition of your organization's tax-exempt status.

3. A financial statement, a signed CPA or public accountant review report, audited financial statement, with Independent Auditor’s Report or UFR (most recent). Mark this “Exhibit Three”.
4. If the organization’s actual percentage of fundraising and administrative expenses is more than 35 percent, include a detailed justification and plan to reduce expenses to 35 percent or less in the next fiscal year. Mark this “Exhibit Four”.
5. If using the 2016 (2015 if 2016 is not filed as of yet) IRS Form 990, copy of face page (page 1) and page 10.

If using a Form 990EZ, a copy of page 1 of your 990EZ AND page 1 and 10 of your Pro forma 990.
Mark these copies “Exhibit 5”.
If the Internal Revenue Service allows your organization to file a Form 990EZ, you must also submit a pro forma IRS Form 990.
Pro forma IRS Form 990 Instructions – The IRS Form 990 (long form) can be downloaded from the IRS website (www.irs.gov). The following sections must be completed: Part I (Summary), Part IX (Statement of Functional Expenses). See application for specific details.
6. Current names and addresses of board members. Mark this “Exhibit Six”.
7. NEW SINCE 2014
All charities NEW and REAPPLYING must provide a signed copy of the non-discrimination policy on the organization letterhead. You can find the policy on our website www.comecc.net. MUST BE EXACT LANGUAGE Mark this copy “Exhibit Seven”.
8. Evidence of provided programs or services. Mark this document “Exhibit Eight”.
9. Indication of local presence. Mark this document “Exhibit Nine”.
10. Electronic version of the 25 word description file in xls format.
The decision to allow an Unaffiliated organization, a Federation affiliate, or a Federation to participate and be listed in a Commonwealth of Massachusetts Employees Charitable Campaign brochure rests with the Statewide Review Committee.
Lack of sufficient information about your individual organization or Federation may result in a decision by the Statewide Review Committee not to include your organization in the Commonwealth of Massachusetts Employees Campaign (COMECC).
All required documents must be submitted before the application deadline. If the Statewide Review Committee requests additional information or supporting documents to verify information supplied in the application form, the individual organization/federation must comply within seventy-two (72) hours of the request.
Missing documents will not be accepted during the appeals process.
Completed applications
and the electronic 25 word Description Information spreadsheet (.xls)
must be received by the
COMECC Campaign Manager no later than
Friday, March 10, 2017, 5:00 p.m. local time.
If you are a Federation affiliate,
send your application and 25 Word .xls file only to your Federation!
