Dear PTA Program Applicant:

Thank you for your interest in the PTA Program at Baker College of Auburn Hills. This packet contains the formal application for the program.

It is recommended that each applicant familiarize themselves with these official Baker College publications for college requirements and course information:

  • Baker College Catalog Supplement and Waiver Form – PTA AAS degree
  • Baker College Graduate and Undergraduate Catalog 2016-2017
  • Baker College Student Handbook (found in Catalog or online)

Please note, all applications for the PTA Program at Auburn Hills must be submittedonline. Applications must be submitted by October 20, 2017 at 5:00 p.m. The deadline will be strictly enforced and the receipt date and time the application is submitted will be recorded. Make sure to keep copies of all completed forms for your records.

Your application and background check will be reviewed after final grades for Fall Semester 2017have been posted, no later than December 18, 2017. You will then receive an email of acceptance or a mailed letter of non-acceptance into the program. Additional information for the new student PTA orientation meeting on January 4th, 2018from will be included in the mail for students accepted into the program. If accepted to the PTA Program, you must attend the orientation meeting on January 4th, so be sure to keep that date open.

Fall 2017: Step by Step Application Process

  1. Application Information including the online link and forms will be availableonline atbetween August 28thand October 20th, 2017here: PTA Application Page. The link to the online application should take you to a Google Form. You will have to sign in with your Baker username and password to access the form. It is located here:

***The Deadline for submitting applications is October 20, 2017 by 5 p.m.***

  1. The ATI TEAS Exam (formerly TEAS V) – dates and process for signing up for exam will be announced by week 10 of the Fall semester at the latest.
  2. Testing will be offered on all Baker College campuses at some point either Week 14 or 15 of Fall. Auburn Hills TEAS dates will be during Week 14 (Nov 27-Nov 31) which is the week after Thanksgiving break.
  3. Payment for the exam will NOT be through the Baker Bookstore, but through ATI TEAS website when you register. The link for the registration will be emailed to you after you apply.
  4. The entrance exam can only be taken once in a 12 month period
  5. If you have taken the TEAS V exam within the past year at another campus, you will need to provide a copy of the results and include it in your application. We cannot access previous results online.
  1. After your application is submitted it will be reviewed and you will receive email confirmation that is was received. The review the application and verification of eligibility will include:
  2. Completion of prerequisite coursework
  3. Science (SCI) coursework completed within the past 60 months
  4. Grade of B- or better in all prerequisite coursework
  5. Verification of PTA major
  6. Verification of official transcripts, if applicable
  7. Update personal contact information if needed
  1. Distribute recommendation forms to three (3) professional references (employers, professors, etc). Do not solicit recommendations from family or friends. References must mail letters back to the PTA Program Director, Dr. Sarah Case, by October 20, 2017.
  1. Complete observation hours in two different settings and turn in Clinical Observation Forms (total 20 hours)to the Health Science Department office, PTA Program Director, Room 209, by October 20, 2017.
  1. All completed applications including letters of reference and observation hours must be received by 5:00 p.m., October 20, 2017. Request a receipt when dropping off materials.
  1. Complete a criminal background check through Castlebranchno earlier than Monday November 6th, 2017. All background checks must be completed online prior to December 16, 2017or applicant will be deemed ineligible.

After applications have been received and processed:

  1. Students will be notified of acceptance status by email after December 18th, 2017 followed by an official letter in the mail.
  1. If you were not accepted, it is recommended that you make an appointment with an advisor or PTA Program Director to discuss your next steps.

PTA Application Checklist

Your PTA application will be complete once you have submitted all of the required documents, including this checklist and background check acknowledgement.

DUE BY OCTOBER 20, 2017

1. _____ Completed PTA Program 2017 Application for Admission formONLINE.

2. _____ Threepurple PTA recommendation forms. Each one in a sealed envelope with the evaluator’s signature across the seal. (Forms submitted from friends or family are not acceptable and will void your application.) Mailed to Dr. Sarah Case.

3. _____ Completed, signed documentation of 20observation hours (2 sites, 10 hours each) on pink forms.

DUE BY DECEMBER 16, 2017

4.______Completed entrance exam. Submit scores to PTA Program Director.

PTA PROGRAM ACCEPTANCE CRITERIA

An objective score is calculated for each student based on his/her GPA in weighted courses, TEAS Entrance Exam results and transferred/repeated/withdrawn weighted credit hours. Total possible score is 100 points. The student score and fulfillment of the additional required criteria determines placement in the program.The points are assigned based on the following:

  1. GPA in weighted courses (65 points total possible)

Each single-weighted course is worth the following points based on the grade received:

  • SCI 211/HSC2510 (4 credits)PTA 111/PTA1110 (2 credits)

A = 7.65pointsA = 3.82 points

A- = 7.07 pointsA- = 3.54 points

B+ = 6.31 pointsB+ = 3.15 points

B = 5.74 pointsB = 2.87 points

B- = 5.16 pointsB- = 2.58 points

Each double-weighted course is worth the following points based on the grade received:

A = 18.06 points

A- = 16.70 points

B+ = 14.90 points
