INSTRUCTION SHEET
This is the instruction sheet you will need to follow when submitting your scholarship application. Please read and complete the information thoroughly. Applications are due in the Extension Office by April 1, of the current year.
1. Please place the completed application in a folder or binder.
2. DO NOT ATTACH OR ENCLOSE THE RULES OR GUIDELINES.
THE APPLICATION MUST BE SUBMITTED IN THE FOLLOWING ORDER:
1. Cover page
2. Juanita Montgomery Whitham 4-H/FFA Renewal Scholarship Identification Data
3. Organization Activity Sheet. This should include any activities or participation that occurred during your year at college
4. Letter of reference from a college Professor/Employer/or Loan Officer
5. Most current copy of college transcript/grade report.
6. A letter from applicant stating future plans and goals.
In order for the application to be considered applicants need to submit ALL of the above information.

RULES AND GUIDELINES
1. Scholarship forms are available from each of the three county high schools, the Crook
County Extension Office, can be e-mailed in Word format by contacting the Crook County Extension Office or downloaded in PDF format at www.crookcounty.wy.gov/extension.html
2. Applications must be typed or printed on a computer. If application is not typed or printed on a computer, it will be disqualified. There will be no personal interviews.
3. Applicants do not have to be enrolled in the college of Home Economics or Agriculture. Applicants must plan to attend an institution of higher education, college, trade school, etc.
4. This is a one-time renewal scholarship for college freshmen. The number of renewals will be based on available funds in the scholarship account.
5. Scholarships will be awarded 40% on 4-H and/or FFA work, 20% general leadership and community service, and 40% on scholastic achievement. The Extension Office will attach a copy of the original scholarship application to this renewal application for the judges to review.
6. Once the scholarship(s) have been awarded, it will be sent directly to the recipient in the fall semester upon verification that the recipient is enrolled in college or trade school. Scholarships can be used for room and board, tuition, books or fees.
7. The 4-H Council President or the chairman of the scholarship committee will assist in the screening of the scholarship applications for eligibility prior to the judging.
8. The Crook County Homemakers Council President will appoint a committee consisting of three (3) or five (5) Crook County Homemakers members. This committee will select the scholarship winners. Committee members cannot have any family relationship to applicants. If there is deemed a conflict of interest the selection committee can be from another county.
9. The Crook County Homemakers committee will score and judge the renewal scholarship applications first by using the score sheet provided by the Extension office. The decision of the committee will be final and submitted to the Extension office.
10. The renewal scholarships recipients will be notified of award by letter from the scholarship committee and recipients will also be announced at 4-H Achievement Day.
