INSTRUCTIONS:
Season: The data you are providing on this form should be for one of the four seasonal periods listed above. Please use separate forms for each season. Forms are due on the 10th of the month following the end of a given season e.g. forms submitted for the Spring Season are due on June 10th.
Observation Dates: For each species recorded during the season enter the date that it was first recorded and last recorded in the appropriate column. You are encouraged to submit the maximum numbers of a particular species recorded during the season, especially those of greatest conservation concern. The maximum date is optional and best used if you feel that you recorded significant numbers of a particular species. It is not necessary to enter insignificant numbers of abundant species in this column, such as Black-capped Chickadee, Blue Jay or American Robin. This information is of little use to the seasonal editors and for bird conservation. Use your best judgment as to what you think is important if you use this column. Also, if reporting maximum numbers please note the highest number recorded in the margin to the right of your max date entry. Finally, please use red ink to underline anything you consider unusual or significant.
Documentation: There are two classes of birds listed on this form that require documentation.
1) Species in Capital Letters: These are species that are rare in Wisconsin and any reports of them should be accompanied by a brief description that supports the identification. This description can be written on the back of this form or on a separate sheet of paper.
2) Species in Capital Letters with an Asterisk: These are species that are extremely rare in the state and reports of them must be reviewed by the WI Records Committee. If any of these are reported on this form a more thorough documentation should be provided. This is best done on a WSO Rare Bird Report although any format is acceptable. When providing this documentation please give a thorough description of the bird and explain why similar species were eliminated. Also provide the date, time and location of the observation along with comments on habitat, vocalizations, distance to the bird, length of observation and the type of optics used.
Please note that these documentations are requested to help the WSO maintain a thorough and accurate historical record of unusual avian species that have been recorded in Wisconsin. Though no one seems to enjoy writing documentations they are an important and essential part of this process. Your help in providing them is greatly appreciated.
