Job PurposeTo lead and develop high quality patient care to meet the Holistic health care needs of the practice populations.
To work within NMC Code of Conduct and to agreed practice standards and protocols.
To supervise and manage the practice nurse team including Health Care Assistants ensuring cover is in place at all times i.e. 8.00 a.m to 6.00 p.m. Monday to Friday.
To ensure that the development needs of the practice nursing team are met
To participate in service re-design as and when requested
The Post holder is responsible for the delivery of general practice nursing to the populations of both the Culbin and Varis Medical practices. The post holder will work collaboratively within the general practice teams to meet the needs of patients, supporting the delivery of policies and procedures and providing nurse leadership as required.
Clinical Responsibilities
Assess, plan, develop, implement and evaluate treatment programmes that promote health and well being.
Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
Work with other health care professionals to diagnose, monitor, manage and treat chronic disease.
Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support and manage health needs of women presenting for family planning and cervical cytology consultations
Implement and participate in vaccination and immunisation programmes for both adults and children
Meet the needs of patients presenting for wound care
To lead the annual flu campaign and other mass population vaccination programmes as needed in line with national guidance.
To conduct family planning consultations and assist the GP’s in providing LARC services
To assist or organise the assistance of staff in the provision of minor surgery
To provide patient health promotion/disease prevention education and counselling focusing on health behaviour change.
To counsel patients on the meaning of test results and other medical information, conducting phone and in person consultations: triaging patients to an appropriate clinician, finishing a line of investigation, or arrange additional investigations as appropriate in line with practice guidelines and protocols and in partnership with clinical colleagues.
Undertake opportunistic screening when clinically required.
Maintain a level of expertise in specific chronic disease management and other pertinent areas of health care, and seek to develop new and improved skills.
Direct, supervise and help mentor nurses and health care assistants in the clinical setting.
Offer comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, food hygiene, sun protection, first aid and emergency medication.
Provide a comprehensive family planning service
Provide healthy lifestyle and smoking cessation advice to individuals and groups.
GENERAL
To maintain your own annual registration with the NMC and adhere to its Code of Professional Conduct
Maintain any other required registrations and certifications necessary to fulfil your role
To promote a professional approach at all times by dress and attitude to patients and staff, both in and out of the Practice, setting an example for those under your direct authority and to the team as a whole.
To participate in and attend practice meetings and in-house training as required.
Contribute to and work effectively within the primary health care team
Advise and support other members of the team aiming towards common objectives.
LEADERSHIP
To create a calendar of appraisal and training for Health Care Assistants
To provide nursing leadership and support in the development of new practice services as needed.
To contribute to the development of policies/protocols/guidelines within the practice
Formulate reports as required for the Partners/ Practice Manager
Promote the continuing professional development of yourself and other nurses & HCA’s within the practice.
Recognise the opportunities for working in partnership with the Primary Health Care Team.
Encourage nursing practice in line with relevant evidence based research.
Promote effective communication and relationships within the team.
Recognises the development needs of others and provides clinical supervision
