B. PURPOSE

The Case Management Intermediary (CMI) seeks vendor agency (ies) to provide case management services as part of a Continuum-wide Disaster Recovery Rapid Rehousing program for homeless individuals and families due to Hurricane Harvey contingent upon full funding. The objective of the Request for Qualifications (RFP) process is to receive from each interested Rapid Rehousing Case Management Vendor a formal statement of qualifications (SOW) to identify the most qualified vendor(s) to provide case management and navigation services to homeless households within the Houston/Harris County CoC. The SOW will be used to evaluate each vendor’s experience, capabilities and qualifications to provide services that meet the needs of homeless families and the objectives of the Rapid Rehousing Collaborative. This funding will support:

-  6 month contract

-  Each case manager will be on an hourly rate of $30/hour not to exceed 40 hours weekly

-  Hiring/re-purposing of staff will need to be immediate or within 30 days. Agreeing to these positions will be agreeing to this timeframe.

C. GOALS AND EXPECTATIONS

RAPID REHOUSING PROGRAM GOALS:

1.  Rapidly move individuals and families from homelessness to housing.

2.  Provide temporary, intensive, housing stabilization case management services to households experiencing homelessness.

3.  Provide short or medium term financial assistance to eligible families.

4.  Coordinate services across the Houston/Harris County Continuum of Care (CoC).

5.  Build relationships with landlords across the Continuum.

6.  Provide stabilizing services and connect to mainstream resources which ensure households remain stably housed.

CASE MANAGEMENT VENDOR EXPECTATIONS:

1.  Execute the Houston/Harris County CoC Rapid Rehousing Collaborative case management and housing navigation funds for the area which includes Harris and Fort Bend Counties.

a.  Case managers will be qualified to effectively provide housing stabilization case management and have the ability to provide and support housing navigation services as needed.

2.  Meet and/or exceed the outcomes below:

a.  80% of households will exit to permanent housing

b.  70% will not return to homelessness within three months of exit

c.  70% of households increase income during program enrollment

d.  70% of participants obtain mainstream benefits

3.  Report on outcomes and identify opportunities for technical assistance.

4.  Each Case Manager will have a caseload of up to 35 families.

5.  Conduct Housing Placement Assessment and develop Housing Stabilization Plan and Participant Budget for homeless families referred through coordinated assessment.

6.  Participate in weekly/bi-weekly meetings with other RRH case managers to troubleshoot and implement the new system-wide RRH system.

7.  The expectation is a rapid return to permanent housing for the client as supported by proper case management and agreed upon Housing Placement Assessment, Housing Stabilization Plan and Participant Budget.

8.  Maintain a willingness to be flexible as a vendor of a system change.

Description of Qualifications:

Case management vendors will be selected on their ability to achieve the continuum wide outcomes and meet the expectations of the rapid rehousing system and ensure compliance with the written standards for rapid rehousing assistance. Case management vendors will be selected on their ability to comply with federal, state and local regulatory expectations. For experienced vendors, prior performance will be taken into consideration.

D. SCOPE OF WORK

Provide a specific Scope of Work (SOW) for this RFP. The length of submittal document shall not exceed 3 pages (single sided). The SOW shall state the organization’s qualifications and experience fulfilling the goals and objectives outlined above.

1.  Provide a brief description of your organizational history and capacity to fulfill the qualifications described above, (date business started, current total number of employees, any special accommodations/services that could be provided, etc.)

2.  Describe, if any, experience operating a Rapid Rehousing Program as a provider. If none, then please explain how your qualifications prepare you to be a case management vendor for the Rapid Rehousing System.

3.  List your trainings, certifications, licensures, and Evidence Based Practices (EBP) that your organization employs.

4.  Describe experience building relationships with landlords

5.  Describe areas where you have specialized expertise (SW Houston, Ft. Bend County, NW Houston, etc.).

6.  Describe your experience as a provider in a multi-agency, collaborative effort. Include examples of work that had shared goals and outcomes, and required organizational shift to achieve those system-wide outcomes.

7.  Describe any experience using the Homeless Management Information System (HMIS).

8.  How many positions can you repurpose/hire within 30 days?
