Dear Parent/Guardian and Future Camper,
Thank you for your interest in Camp Discovery. We are looking forward to your time with us. There are some Wisconsin state regulations which affect the medical aspect when staying here at Shepherds. These are things that need to be done four-weeks beforethe first day of attendance in order for their registration to be confirmed.
- A complete physical within 1 year of the first day of his/her stay is required and must be signed and dated by your physician.This physical must include the following:
- A TB skin test with negative results. If unable to have a TB skin test, one of two other options are acceptable; a TB blood test or a chest x-ray with negative results. Date must be included.
- Tetanus shot if not given within the last 10 years. The date must be included.
- Hepatitis B shot, series of 3 injections,must be started, preferably completed. Dates of the injections must be included.
- The physical should not have any abnormal readings other than what is normal to the applicant.
- A current and complete list of all prescription and non-prescription (OTC) medications including: vitamins, nasal sprays, topical medications,etc... must be signed and dated by your physician. The following is not only our protocol but Wisconsin state law as well:
- Our staff will not be able to use anything that is not on the signed list of medications.(All medications will be administered based on the prescription label, so pleaseinform their doctor of any changes made at home so they can write a new rx).The doctor’s prescription needs to match the pharmacy’s directions because we will follow the doctor’s prescription first.
- All medications need to be sent in the original prescription bottles.
- Our medication pass times are: 7AM, 8AM, 12NOON, 4PM, and 8PM; to avoid mistakes we like to keep the camper’s medications in the Shepherd’s medication pass times. This includes any over-the-counter medication.
- We will supply anything listed on the PRN list, including Tylenol or ibuprofen. If he/she needs a PRN medication it will be given when they ask staff. Your camper is not allowed to have any kind of medication in their room, including mouthwash, medicated shampoos, medicated lotions or creams, etc...with the exception of personal inhalers.
The above information must be to Shepherds 1 month before Camp Discovery, as this will ensure that all the information required by the state of Wisconsin is in place and accurate. If the information is not complete, your registration may not be approved and the camper may not be able to attend this year.
