Position Purpose/Job Summary
The Midwifery@ Home Midwife functions in accordance with legislation, common law, health standards and policies pertinent to nursing practice.
The Midwifery@ Home Midwife practices in accordance with Professional Nursing standards and is guided by the Code of Professional Conduct and Code of Ethics for Nurses in Australia.
The Midwifery@ Home Midwife is a vital member of the multi-disciplinary health care team who collaboratively strives to deliver excellent care to patients and their families.
The Midwifery@ Home Midwife has a clear understanding of the importance of providing nursing practice, which focuses on the best possible outcomes for their patients.
The Midwifery@ Home Midwife interaction with patients, relatives and colleagues is guided by the organisation’s values of patient centered care, trust and respect in an environment that continuously identifies opportunities for improving the quality of patient care.
The Midwifery@ Home Midwife effectively and safely delivers patient care and also delegates care to other members of the health care team within the scope of practice of individual team members.
Midwifery@ Home Midwife will provide safe and effective patient-centred midwifery care in the community to antenatal/postnatal women and their babies. To work collaboratively with members of the multidisciplinary health team in the planning, implementation, assessment and evaluation of care within the maternity unit. To ensure optimal outcomes for patients/clients and carers of the service through the promotion of best practice including the appropriate escalation of care .To effectively manage resources relevant to the service.
Education, Qualifications & Special Training
  • Current registration with Australian Practitioner Regulation Agency as a Registered Midwife
  • Demonstrated experience and clinical skills in midwifery.
  • Demonstrated commitment to women focused care
  • Demonstrated effective communication skills, both written and verbal
  • Relevant computer skills, i.e. word, excel, power point
  • Demonstrated commitment to professional development
  • Experience in community based midwifery and understanding of the concept of model of midwifery care
  • Current NSW Drivers Licence
  • Ability to work as a member of a multi - disciplinary team
  • Knowledge of WH&S guidelines

  • Community health, counseling, lactation, or immunisation certificate/experience or willingness to acquire

Role Accountabilities & Key Performance Indicators (KPI)

It will be necessary for the Registered Nurse / Registered Midwife, at the time of their professional performance review, to be able to demonstrate an understanding of the concepts / duties / responsibilities of each criterion.
Demonstration of the key performance indicators (KPI) may be in the form of discussion with their manager, observation of their work practices and feedback from supervisors or appropriate personnel eg After Hours Nurse Manager, Clinical Nurse Consultant / Clinical Nurse Educator.

CLINICAL PRACTICE
Working within primary health care and Families NSW frameworks and utilising current best practice:
  • provide primary health care, health assessment and
health education within a multidisciplinary environment
  • provide Midwifery@Home nursing service, which encompasses screening, assessment, intervention, support and information sharing, to babies and their parents/carers during the post partum period
  • work in partnership with families to optimise their physical, social, emotional and intellectual well-being
  • facilitate relevant self-help and support groups relating to parenting issues e.g. breastfeeding
  • provide information to families and individuals about relevant resources and agencies and refer as appropriate
  • assist families to establish and maintain supportive relationships within their community
  • promote and support breastfeeding in accordance with the WHO code and NSW Health/HDHS guidelines/policies
  • participate in the development of strong and collaborative links with local agencies and service providers to ensure appropriate integration of Families NSW and partnership approaches to improving the health status of families and the community
  • adapt and develop identified innovative clinical practice models with other clinicians and field staff
