*Please note that all tuition must be paid in full by July 1st or a bank loan be secured for amount due!!!!! If you request that your bank loan be paid back over 12 months, you must close your loan by April 29, 2011, which means that your first payment for the new loan will be due in May, 2011. Otherwise, all bank loans will have to mature by March 30, 2012.

1.  This application MUST BE RETURNED to the School Finance Office as soon as possible, but no later than 3:30 p.m. on Thursday, April 14, 2011. We can not accept any after this date.

2.  Application must be completed in full, with all required attachments (see below), for consideration.

3.  You may, if you wish, attach other letters or information that will help to determine need.

4.  Tuition Assistance is NEED-BASED.

5.  It might be beneficial to secure a recommendation or letter from your Pastor.

****REQUIRED ATTACHMENTS****

1.  Please attach a copy of your 2010 Federal Income Tax Return and copies of your W-2 and/or 1099.

2.  If you own a business, please provide a copy of the Profits and Loss Statement for Year 2010.

3.  Please attach an itemized list of monthly expenses.

4.  It is recommended to have a letter of need from your Pastor. (St. Mary and Assumption Parishioners do not need this.)
