CONDITION OF ACCEPTANCE - SOR TO GA

Dear :

Welcome to SOR!

It is our pleasure to welcome you to the Service Option Ready (SOR) phase of our roadmap to the successful implementationof your managed services solution. Champion will assist you with the transition to the full General Availability (GA) state during the coming days. The items associated with this effort are listed on page 2 of this document.

Please note that, during the transition process, issues regarding your hardware or software environment will be handled by our technical team in a prompt manner. However, please understand that, prior to the GA phase, the highest priority (Priority One) support tickets and the full response of the OperationsCenter staff are not authorized. Priority One support tickets will only be authorized when the environment is in a General Availability (GA) status. We will work together with you to make this transition as smooth and as efficient as possible.

When the SOR phase begins, the control of the environment will be turned over to you, the Customer, for the purpose of loading applications during theCustomer Production Ready Date (CPRD) and Customer Production Date (CPD) transition. The control of the environment will be returned to Champion Managed Services before the final General Availability (GA) phase.

During this SOR phase we will be exchanging information and documents that will ensure the successful completion of the phase, and will allow us to proceed to the General Availability (GA) phase. This document outlines responsibilities and deliverables that are associated with the advancement and completion of the SOR phase.

There are responsibilities and deliverables required from the service provider, IBM/CMS, and the Customer. The related information provided in this letterincludes:
