Community Short Breaks Summer Holiday Activities 2017

Dear Parents and Carers

The Booking Hotline for the Summer Holiday activities will be open from Monday 17th July until Friday 21stJuly from 09.00am until 4.00pm.

Bookings can also be made online at this period

Bookings either online or by telephone requested outside of the dates stated will not be processed.

Please note places in sessions are limited; you will be able to book your child or young person onto a maximum of threesessions each week, you can also put your Child on the waiting list for one other session.

If you have made a booking and your child or young person is not able to attend, please can you let us know as there maybe others who are on a waiting list?

If your child or young person will be attending with a support worker/personal assistant, please let us know at the point of booking as this will impact staffing levels and places in groups.

Sessions will be delivered for children and young people aged 5-11 or 12-18 unless stated otherwise. Please find this information below next to named activity.

If your child or young person has never attended a short break activity, we will require a completed short breaks passport and DLA letter. Please let us know at the point of booking if you require this information.

Please note we cannot support siblings in activities unless stated.
