Welcome by Mike Wilcox. Introductions were made.

Updates:

Allen and Julie/Wadena: active program for 2+ years. A couple of problems since last quarterly meeting. Look at wide variety of patients. Work with hospital discharges or from clinics. Try to keep patients out of nursing homes. Clinic visits have gone up. ER visits and admissions are down. They do meet readmission program. Working with CentraCare. Biggest problem is that they are independent hospital but fall under CentraCare EPIC. Administrators are involved to assist. Consent forms for CP services created. Group home patients – need to send consent to POA. Future equal reimbursement for telemedicine program. Especially for jail patients and after stroke care. Started working on asthma core measures. CPs do part of care coordination after hours. A lot of push back from CentraCare related to EPIC to add CP build and is expensive. CPs are well trained in safety (regarding welfare checks, vulnerable adult etc.). Have done a lot of advertising for the program. They work well with home care.

Corey/Allina: started program 2 ½ years ago. Getting pulled in multiple directions. Focusing on mental health, reduction of hospital readmissions. Working with hospice and hopefully home care. Hospital readmission patients focus on CHF, COPD and sickle cell. System wide for all 13 hospitals hope to spread the program. Also have an EPIC issues (resources and finances). Without EPIC can’t schedule, bill, mine data.

Jennifer/Northfield: taking baby steps. Started meetings about a year ago, sidelined with administration but now on board because of readmission discussions. Talking about tying them into discharges. Hope to be up and running in 2016. Has come up with a template for documenting and will then scan to the hospital and send copy to physician if needed.

Amber Brown/HCMC: now have a primary care physician who provides medical direction with Brian Mahoney. Have a template for documenting and also issues with EPIC. They can now see MN visiting nurse documentation which helps reduce dual referrals. Working on referral flow chart. Will provide summary for the group.

Pete Tanghe/HealthEast: working on getting into primary care clinics for behavioral health. Expanded to populations with patients that need interpreters. Were able to get EPIC to create a CP template. He will write a summary of their experience and send to this group. Builders want to build something which may be more than what is needed for EPIC CPs. Working on finalizing electronic protocols which he will also forward. The benefit of their pilots is becoming evident quickly.

Mike: MDT foundation provided a grant to focus on access to health care within that county. Embedded in free clinic collaborative. CP, CHW to move out into the county especially for immigrants and geriatrics. Will partner with Northfield. 2016 telecommunication funds may be available state and federal initiatives with CPs. SIM initiatives focusing on front line health care workers to afford benefit to health care access in various parts of the State. Concerns about EMR piece.

Mike stated the importance of developing a common set of data points to provide to health care organizations and third party payors to demonstrate the value of CP programs and for reimbursement.
