JOB DESCRIPTION

POSITION TITLE:Case Manager– Pathwaysto Home

POSITION OBJECTIVE: This position provides direct client case management for families or individuals who meet the criteria for the Pathways to Home (PTH) program. This project position is associated with the implementation of the Rapid Re-Housing grant that was awarded by the Homeless Services Network.

ESSENTIAL FUNCTIONS:

  • Provide case management for a minimum of 15-20 families in the PTH program using the Rapid Re-Housing model of practice.
  • Support and mentor family members as they develop and implement their Family Case Plan and Safety Plans (when applicable), meeting a minimum of once per month with each family.
  • Ensure direct delivery of services to meet the needs of program participants, as often as necessary.
  • Process funding authorization requests and certification documentation in an accurate and timely manner.
  • Assist program participants with the necessary supports and referrals to obtain and maintain stable income and housing.
  • Monitor, evaluate and document program participant progress as required in the PTH program.
  • Utilize required database and reporting programs within specified time frames and in accordance with data accuracy policies.
  • Participate in case review meetings, staff meetings, on-going training and mentoring to assure fidelity of the program’s case management model.
  • Participates in creation and annual reviews of Unit Policy and Procedure Manuals to ensure all policy and procedures reflect current state, federal and system of care requirements.
  • Participates in the agency strategic plan & quality improvement processes.
  • Ability to safely & successfully perform essential job functions consistent with the ADA, FMLA, & other federal, state, & local standards, including meeting qualitative & quantitative productivity standards.
  • Ability to maintain reasonably regular, punctual attendance consistent with ADA, FMLA & other federal, state, & local standards.
  • Ability to comply with all personnel policies & procedures.

QUALIFICATIONS:

REQUIRED:

  • Bachelor’s Degree from an accredited college or university and two years of case management and support experience; or an Associates Degree from an accredited community or junior college and four years of case management and support experience.
  • Experience working in a non-profit organization.

PREFFERED:

  • HUD housing knowledge
  • Knowledge and understanding of prevention initiatives

SKILLS:

REQUIRED:

  • Must be proficient in Word, Excel, and Outlook.
  • Must be able to learn web based tracking tools.
  • Ability to work effectively with other professionals and non-professionals.
  • Highly sensitive to the needs and welfare of children and families.
  • Highly self-motivated and able to work independently when required.
  • Excellent interpersonal skills, written/verbal communication and presentation skills.
  • Must be exceptionally well organized and be capable of completing assignments with minimum guidance and supervision.
  • Demonstrate cultural and linguistic competence & sensitivity to population served.
  • Ability to travel.

SPECIAL CONDITIONS:

  • Thorough Background Screening upon hire including FBI, FDLE, Local Law check, Employment Reference check, DMV check, Criminal Records check, Social Security Trace & Drug Screening. FDLE check will be run at least every 5 years.
  • On-call assignments- Yes
  • Travel- local: 50% per week, long-distance: 1% per month
  • Attend at least 15 hours of in-service training annually
