Deanne Calisthenics Club (Deanne) has a very successful and proud history and we’re looking forward to another great year.
This yearDeanne will be participating in 3 local competitions and our Subbies through to Senior teamswill also travel interstate for their final competition of the year. These competitionsare a wonderful performance opportunity for all of our members and are a real highlight for everyone involved. As always we will close out the year with our annual concert and presentation in November.
Calisthenics is a competitive team sport that requires all girls to competein all competitions.It is important that you understand this level ofcommitment is required. We respect and highly value our coaches whose dedication and commitment enables Deanne to compete at a high level in all competitions.
Our fees cover around 80% of ourexpenses each year and we have a number of social events and activities throughout the year where we all pitch in to help raise additional funds for the club and teams.
We ask you to help by assisting withraising funds through our major fundraisers; performing a duty or 2 at our own Deanne Friendly Physicals Team Competitionand at anACT competition; helping to create and maintain our costumes and stage props, and assisting with other activities such as fundraising BBQs for your team.
For classes: participants need a black leotard, tights or leggingswith hair up and an aesthetic skirt (provided).
Registration
Please complete pages 3 and 4 of this document and complete any other relevant forms required and return by the first lesson for returning members and by the end of the second lesson for new members.
Relevant forms can be completed and returned in one of the following ways:
- attend the registration day at St James Church in Curtin on Thursday 1st February 2018 from 4.30 to 6.00pm, registration forms will be available or bring your completed forms with you; or
- email to ; or
- complete and print paper copies of all forms and return to your Team Manager when 2018 classes commence.

FEES
- You will receive an annual invoice for 2018 with adjustments for individual fundraising or incidentals (ie merchandise purchases) made throughout the year.
- You have the option of paying annually, each term (by week 3) or by regular agreed instalments. Direct deposit into our account is recommended and preferred.If paying by instalments please complete the Direct Deposit Schedule and return with the registration form.
- Your fees cover all classes, participants competition entry fees & costume hire.
- All outstanding fees from the previous year must be paid before registration can be accepted.
- A family discount for siblings is available. Fees can be split between families. Please complete and submit the family discount or split payment form.
- Note: If any fees remain unpaid after the due date recovery proceedings may be instigated and the costs of these proceedings will be added to the debt.

ACKNOWLEDGEMENT AND CONSENT FORM FOR REGISTRATION OF PARTICIPANT IN 2018
Y/NFees: I acknowledge and agree to pay the relevant fees for my child/children or myself. I understand that if the club has to recover unpaid fees and charges the relevant costs will be added to the debt.
Additional Costs: I understand that there may be, and agree to pay, any additional costs which may occur during the year for activities such as Deanne social activities (eg. team bonding activities, sleepovers), and for personal items such as flesh underwear & team uniforms, dance shoes (seniors), hairpieces & make-up if required.
Interstate Competition: I acknowledge and consent to my child/children/myself attending the Interstate Competition and agree to the additional financial commitment required for this activity. (All age groups except Teenies and Tinies)
Clubs and Rods: I understand and agree to pay for lost or damaged clubs and/or rods issued to my child or myself at a charge of $40 per pair of clubs (even if only one club is lost or damaged) and $30 per rod.
I understand Aesthetic skirts are required for all participants and incur a cost of:
Tinies/Subbies: $10 hire fee per participant / per calendar year. $20 replacement fee.
Juniors/Inters/Seniors: 1st skirt and any lost/damaged training skirts will be issued/replaced at a cost of: Juniors $30.00 or Inters/Seniors:$35.00
Fundraising: I understand that fundraising organised by Deanne, while voluntary, is for the benefit of the whole Club and that my child/children or my participation will be requested during the year.
Medical Consent: I consent to medical treatment and ambulance transport being sought in an emergency, either for my child/children or myself, and I agree to pay any costs incurred.
Right to use Image: I acknowledge and consent to photographs and electronic images being taken of my child/children or myself during participation at any Deanne Calisthenics activities. I acknowledge and agree that such photographs and electronic images may be used by Deanne Calisthenics for promotional or other purposes without my further consent being necessary. Further, I consent to Deanne Calisthenics and CaliACT using my/their name, image, likeness and also my/their performance in calisthenics activities, at any time, by any form of media, to promote calisthenics activities.

Make-up & Accessories: I understand that stage make-up is a compulsory feature of calisthenics when performing at various competitions and public displays. Participants may also be required to wear false eyelashes and dance tights during the calisthenics year.
Tanning: I understand and agree to the use of tanning products for body tanning (legs/trunk/arms) at all competitions during the year. (Not Teenies and Tinies)
Uniform:I acknowledge that the Club Jacket is an essential uniform item for all members (complimentary for new recruits only – 1st year) and will be needed all year. Other merchandise items are available and team requirements will be advised by relevant team Managers/Coach at start of given year.
Working with Vulnerable People Act: I understand that if I’m 16 years or over to volunteer for a Committee position or assist within a calisthenics team, that I will be required to hold a Working with Vulnerable People card and produce it on request. (Working with vulnerable people registration – Free for volunteers)
Code of conduct: I understand that there is a code of conduct for participants, coaches, parents and volunteers covered by the Australian Calisthenics Federation and agree to abide by these rules.

[1] Juniors through Seniors purchase their own aesthetic skirt at a cost of $30 for Juniors and $35 for Inters and Seniors.
[2] The interstate competition costs are generally reduced by your fundraising efforts to a total annual cost of around $575 for 2 nights or up to $700 for 3 nights - this includes all transport, meals & accommodation. Your team manager will be able to give you more details if required.
