Guidance for completing the Private and voluntary hospitals (PVH) webform

This document contains guidance and tips on completing the Private and voluntary hospitals (PVH) webform.

You may find it useful to print a copy of this document for future reference.

Important information

·  The company that the Care Quality Commission (CQC) has commissioned to host this webform has changed, and while the design of the form has been updated, the questions in it have remained the same.

·  You must complete the webform for each organisation you are responsible for.

·  The webform may time-out if it is left idle for too long which may cause you to lose information you are working on.

Opening the form

Depending on the security settings on your browser, you may be asked to enable pop-ups after you click on the link to the webform.

If this happens, a yellow bar will appear at the top of your screen. You must right click on the bar and select ‘Temporarily allow pop-ups’.

Logging into the webform

·  You must enter your login ID and password (this will be the same login details you have used for previous submissions).

Navigating through the webform

·  Use your mouse press the ‘Tab’ key on your keyboard to move between questions on the same page.

·  Use the ‘Next’ and ‘Back’ buttons at the bottom of each page to move between pages.

Answering questions

·  Page 1: you will be asked to select the organisation you wish to return data for.

·  Page 2: you will be asked to check your contact details.

·  Pages 3-8: you will be asked to submit data against a number of indicators.

Pre-populated fields (pages 1-2)

·  The form is pre-populated with (1) the name of the organisation you are responsible for (2) your first name and surname (3) e-mail address and (4) telephone number.

·  You must select one organisation from the dropdown list on page 1.

·  You can edit the fields displaying your name, e-mail address and telephone number.

Entering data into the webform (pages 3-8)

·  Data should be entered as numeric values.

·  Enter ‘N/A’ if a question is not applicable to you (do not enter ‘nil’, ‘zero’, ‘999’ or any other special character such as a dash, asterisk, percentage symbol or full stop).

·  There is a limit of five characters in each field.

·  All questions are mandatory (a reminder will appear on the form if any are left blank).

Saving your data

·  You can save the webform and return to it anytime by clicking on ‘Save and exit’.

·  You should then follow the simple steps to receive a URL which you can use to retrieve your form at the point you exited it.

Submitting your data

·  Please do not submit your form until you have completed all the questions.

·  You will automatically receive a copy of your webform via email as soon as you submit it.
