Department:Home Health CareReports to: Clinical Nurse Manager

Job Summary:Under the direction of the Clinical Nurse Manager – coordinates all referrals, completes necessary screening of referrals, performs insurance authorizations and updates, assigns assessments, and assists with hospital discharge planning of home health clients. Assists manager with quality indicators, tracking and analysis. Assists with marketing of agency services and education to referral sources.

AGE OF PATIENTS SERVED:

 Neonatal (Birth to 1 Month) Adult (18 Years to 64 Years)

 Infant (1 Month to 1 Year) Geriatric (64 Years Plus)

 Child (1 Year to 12 Years) All Ages

 Adolescent (12 to 18 Years)  Not a clinical position / not applicable

Qualifications:

Education: Must be a graduate of an approved school of nursing and registered to practice nursing in the State of ______.

Experience:Minimum of one (1)year experience as a registered nurse, preferably in an acute care facility and two (2) years experience in home health. Must be able to make independent nursing decisions and demonstrate critical thinking skills. Must have exceptional organizational and documentation skills. Time management and computer skills essential.

Duties and Responsibilities –Essential Functions are bolded:

  • Answers phone, fields calls, and accepts referrals and telephone orders
  • Coordinates all intake calls and screens for appropriateness
  • Evaluates staff workload and assigns referrals as indicated
  • Verifies insurance with authorizing insurance agency
  • Coordinates patient care with other services; e.g., infusion companies
  • Enters patient referral information into database
  • Assists with quality indicators, collection and analysis
  • Markets home health services and educates referral sources
  • Demonstrates commitment to the department’s goals and to the organization’s mission, vision, and values.
  • Adheres to nursing profession discipline code of ethics.
  • Fosters positive internal and external customer relations.
  • Demonstrates priority setting and time management capabilities.
  • Fosters effective work relationships through conflict resolutions, constructive feedback skills, and problem solving abilities.
  • Performs any other duties requested by the department manager.
  • Will maintain confidentiality of all patient related information.
