INVITATION FOR WORKERS’ COMP PROPOSALS

The Lewis County Board of Education, Vanceburg, Kentucky, invites you to submit a proposal for insurance coverage in accordance with the detailed specification and general conditions of this invitation. The workers’ compensation insurance will be for all Lewis County School employees for the time period of 7/1/2017 to 6/30/2018. Proposals must be mailed or delivered to Nick Adams; Lewis County Schools, 65 Central Elementary; Vanceburg KY 41179 no later than the specified closing time. All proposals should be identified with the above reference and must be sealed. Additional copies of this invitation for workers’ compensation proposals may be obtained from the address above prior to the time and date specified by calling (606)796-2191.

TIME AND PLACE OF PROPOSAL OPENING

Proposals will be reviewed at the Lewis County Board of Education, 65 Central Elementary; Vanceburg, Kentucky,April 27, 2017 10:00 AM. All proposals must be received by the time designated and none will be considered thereafter. Failure to have the proposal in prior to closing time will automatically prevent consideration of the proposal. The Board of Education cannot assume the responsibility for any delay as a result of failure of the mail to deliver proposals on time. Any interested parties may attend the proposal review. No immediate decision will be rendered concerning the proposals submitted.

GENERAL SPECIFICATIONS

  1. All proposals shall be valid for a period of sixty (60) days from the proposal review date to allow consideration, study and acceptance by the Board. A company may withdraw the proposal after it has been submitted only if a written request is submitted and received prior to acceptance by the Board.
  1. The company agrees that if a contract should be awarded based on the proposal submitted, the vendor shall protect, defend, and hold harmless the Board of Education, its officers and agents from any suits or demands for payment that may be brought against it for the use of any patented material, process, article, or device that may enter into the manufacture, construction, or form a part of the work covered by either order or contract, and further agrees to indemnify and hold harmless the Board of Education from suits or actions of every nature and description brought against it for, or on account of any injuries or damages received or sustained by a party or parties, or any of the acts of the contractors, his servants or agents.
  1. KRS 45A.455 prohibits conflicts of interest, gratuities, and kickbacks to employees of the Board of Education in connection with contracts for supplies or services whether such gratuities or kickbacks are direct or indirect.
  1. KRS 45A.990 provides severe penalties for violation of the laws relating to gratuities or kickbacks to employees which are designated to secure a public contract for supplies or services.

  1. No proposal can be corrected, altered, or signed after being opened. All prices and quotations must be in ink or typewritten. No pencil figures will be accepted. Mistakes are to be crossed out and corrections inserted adjacent thereto and initialed by the person signing the proposal.
  1. The Lewis County Board of Education reserves the right to waive defects and informalities in proposals and to reject any and all proposals as may be deemed to be in its best interest. A member of the firm that can legally bind the firm must sign the proposal.
  1. If proposed coverage is through a self-insurance fund; a current financial statement and full disclosure of all reinsurance arrangements must be disclosed with the submitted proposal.

  1. The contract period shall commence July 1, 2017, at 12:00 A.M. for 1 year.
  1. The proposal is encouraged to include options for two 1 - year renewals.
  1. Any and all applicable taxes must be included in the premiums. Proposals may be rejected if incorrect calculations are found.
  1. The carrier shall furnish to the Board of Education quarterly reports of all claims incurred and the most recent rating of carrier.
