Before completing this form please note the following:

a)  A Fellow can normally be associated with only TWO NEW nominations for Fellowship in a given year

-  An exception exists for new nominations of female candidates and candidates from outside academia – here there is no restriction on number.

-  This restriction also does not apply to nominations for Honorary and Corresponding Fellowship

-  Existing nominations in their second or third year of consideration do not count towards this limit

b)  Sectional Committee Conveners cannot act either as a Proposer or an Assessor of a nomination.

c)  Members of Sectional Committees are now eligible to act as a Proposer or an Assessor for any nomination.

d)  Council members not directly involved in the decision-making process (Trustees and Observers not also on the Fellowship Committee) are now eligible to act as Proposers and Assessors (no other Council members may act as either a Proposer or an Assessor).

e)  It is helpful to have Assessors from several different institutions to show the breadth of support for the candidate (and only one of the Assessors or Proposer can be from the same institution as the Candidate)

f)  Sectional Committees value these assessments in making their final recommendations, so they should be as fully as possible. Assessors’ reports which contain limited amounts of information, however positive, are not useful to the sectional committees.

g)  This report should focus on accomplishments that you can personally attest to, place them in context, and explain their importance. If you can, include more, or additional, specifics than are in the nomination. Please do not merely repeat portions of the nomination.
