I am not currently attending any high school. I will abide by all of the rules of CentralHigh School and the CentralUnifiedSchool District, including dress code. I understand all of the Activity Guidelines and will follow all rules. A copy of my Driver’s License or other acceptable photo ID is attached.

NOTE: Students who have been involuntarily placed at a ContinuationHigh Schoolwill not be allowed to attend.

Please return this completed permit to your Vice Principals’ Office promptly and leave for administrative approval. You must present this signed permit to purchase your bid to the activity.

NOTE: Central High student and guest must present ID’s at the activity. Central High administration reserves the right to revoke this guest privilege at anytime prior to or during the activity.

ACTIVITY GUIDELINES

  1. The guest will obtain signature from their home school. The CHS student will complete the guest pass and return it with a copy of a photo ID to the Vice Principals’ Office for final signature. GuestPassmust be signed by a Vice Principal before tickets to activity may be purchased.
  2. Along with the ActivityGuestPass, the guest will present a photo ID. The guest may use school ID, driver’s license, or California ID.
  3. Last day to turn in GuestPass to Vice Principal is five days prior to event.
  4. Inappropriate dancing will not be allowed. The first offense will result in a verbal warning. The second offense will result in the student and guest being sent home.

IF THE CHECK AT THE DOOR IS NOT VALID, THE COUPLE WILL NOT BE ADMITTED.

Any violation of school rules, the CHS student’s parents will be contacted to pick up the CHS student and guest. If unable to make parent contact, the CHS student, and any guest found to be under the influence will be turned over to the police. The CHS student upon return to school will be suspended. The other student’s home school will be notified and may be suspended.
