2.Service

The new specialist substance misuse service is for young people up to the age of 24 and their families/carers. The aim of the service is to provide a full range of support and therapeutic interventions for young people using substances at a level that affects their functioning. The service will meet young people’s substance misuse needs and target identified vulnerable young people to enter and exit treatment in an agreed and planned manner, ensuring positive outcomes for the young people and their families. The service will also support and enable universal children’s services to address early intervention and prevention.

3.3Responsible for developing close and effective working relationships with key

agencies and partners to facilitate a planned and co-ordinated multi-agency response to the ETE and family needs of identified Young People.

  1. Main Duties

4.1To be a member of the specialist substance misuse team for young people

4.2 To carry a caseload of vulnerable young people and/or parent/ carers of

young people undergoing treatment.

4.3 Liaising with Youth Provisions, Universal Services, Social Care and Job

Centre Plus services; informing them of the work of the service, and giving

information and advice on substance misuse issues.

4.4 To identify, design and deliver and evaluate targeted outreach drug education workshops to vulnerable young people in school and non school settings.

4.5To develop and oversee a weekly drop in for young people/families and carers.

4.6To develop substance misuse workshops to access parents/carers across the borough. To increase awareness, signs and symptoms of drug use. To enable parents/carers to support the achievement young people are undertaking.

4.7To carry a caseload of allocated young people/parents/carers.

4.8If a young person self or is referred by parent/carer will undertake the CASA, and may take on the lead role.

4.9To offer Information, advice and guidance to the young people at drop-ins across the borough.

4.10To undertake comprehensive assessments of young people referred to the service within 5 days, leading to the development of a multi-agency care plan with SMART targets and identify ETE needs to specialist services where necessary.

4.11To offer appropriate therapeutic treatment interventions including psychological, family support, specialist harm reduction and referral on as required.

4.12To liaise with key partner agencies to ensure all care plans are holistic and meeting all identified needs. To ensure agencies work positively together avoiding duplication or confusion of roles.

4.13To regularly review the care plan and on completion of the treatment to develop an aftercare and tracking plan for each young person.

4.14To involve the young people as much as possible in their assessment planning and delivery of their own service and to feedback on the service as a whole.

4.15To complete all data and performance information required timely and accurately.

4.16To work flexibly, supporting evening and week ends drop in and targeted outreach sessions when required.

4.17To operate within the framework of legislation, national guidance and local procedures to ensure the provision of a quality service to young people with significant substance misuse needs.

4.18To provide Duty cover under the guidance of the Manager as required.

4.19To ensure that adult or child safeguarding concerns and risk management issues are brought to the attention of the Manager in line with procedures.

4.20To maintain an interest and good working knowledge of benefits, training/employment, housing and health issues affecting young people.

4.21To attend and contribute to Team and Service meetings as appropriate.

4.22To attend and be committed to ongoing regular performance and development meetings and annual contribution reviews.

4.23To promote User involvement and feedback at all times as part of day-to-day work.

  • Good basic standard of education.
  • Minimum of S/NVQ level 3in health and social care with relevant DANOS options or equivalent (see above)
