Call to Order / The meeting was called to order at 8:40
Introductions / Special Welcome to HVCC!
Care Coordination in the news / No featured CHW today, next meeting calling for requests.
Announcements / Tiziana Rota (SCCC), Keshana Owens-Cody (HCDI) and Leeza Wood (AHEC) announced a Save the Date for the first CHW Conference in the Capital Region. Date is October 20, 2017, details to follow.
Keshana announced HCDI Newsletter to feature Care Coordination in the next issue. Upcoming meetings will discuss what the group would like to share with the community.
Larry shared information on HCDI Consumer Advisory group, and asked the group to share any questions they may have, and/or want feedback on from consumers.
Training and Development / Care Coordination Task Force participated in a survey in between meetings in regards to onboarding required and/or recommended trainings. Trainings described were a compilation of Office of Child and Family Services and Office of Mental Health Waiver Service Provider “required” trainings. These trainings included: mandated reporter, CPR, first aid, documentation, communication, community safety, healthcare promotion, stages of human development, etc. Employers shared that majority of the described trainings were important but not required (CPR, first aid, and human development). Trainings deemed mandatory were: motivational interviewing, documentation, and cultural competency.
* Some organizations have grants that provide additional required training that is not open to the public.
*Local organizations that provide training are being sought to provide resources to employers with limited training personnel.
(Sidney Albert Training Institute (Parsons), Planned Parenthood, SCCC, AHEC, HVCC, Department of Criminal Justice, Eddy, City Mission, USCRI). / Invite described training organizations to next meeting March 24, 2017 to provide updates on their various programs.
Referral Resources / Keshana provided the Task Force with a spreadsheet of Emergency Resources, as well as Economic Stability resources for those in the region. The group will review resources found, and approve them by March 3, 2017. / Resend resources to group on 2/27/17.
A.L.I.C.E / Keshana and Ann DiSarro presented the A.L.I.C.E NYS Summary to the task force. Keshana described the report as a glimpse into the struggles of the working poor. Keshana encouraged the group to read the summary, and provided additional links to see the full report.
* Questions were raised in regards to United Way’s plan to use the report in our communities.
Additional Follow Up / Meg Wallingford asked if there was an assessment tool that CHWs can use to identify patient needs. Keshana shared information on the CANS-NY (Child and Adolescent Needs Assessment) which is being used in Children’s Health Home. Keshana sent the Adult ANSA tool to the group for review of Strengths and Needs of Adult patients.
*Alliance needs an assessment tool by end of March 2017.
