Student Directory Information Form 2016-2017

Each year we publish a Student Directory which is a valuable communication tool for CCHS parents, students, teachers, and administrators. It contains an alphabetical listing of all current students including grade, address, phone number or e-mail address, and the names of parents/guardians. Also, there is a list of faculty, staff, and other important school numbers.

All students are listed in the directory unless parents/guardians contact the Student Affairs Office at (856) 663-2247 ext. 124, or by returning the “Opt-Out Form” below. If you were published in last year’s directory we will update your child’s grade, but keep the same contact information; however, if you need to make a change to the information we have on file, please complete the section below labeled Update My Information in the Student Directory.

All requests for updating your contact information or “Opting-Out” must be received by JULY 15, 2016.
