Transitions Update

Four lead organizations officially began implementation on 9/24/08: Home Health Care Management, Inc. (HHCM), Independence at Home (IAH), Independent Living Resource (ILR), and Westside Center for Independent Living (WCIL). These organizations are creating a necessary “transition infrastructure” in the State by:

1) making nursing facility residents aware of programs and services that could support their return to the community; 2) identifying residents who wish to transition through use of the Preference Interview Tool; and 3) training transition coordinators to work with residents who wish to return to a community living arrangement.

Nineteen individuals were transitioned between 12/12/08 and 2/26/09, and 75 potential demonstration participants are in various stages of the transition process.

General characteristics of those transitioned:

·  Ten women between the age of 41 and 67;

·  Nine men between the age of 49 and 67;

·  Eleven people with developmental disabilities; and

·  Eight people with disabilities, not developmental.

·  Thirteen people moved into apartments and six moved into a house or duplex.

Four people enrolled in the NF/AH waiver; 11 enrolled in the Developmental Disabilities waiver; three are being supported through In-Home Supportive Services only; and one enrolled in the Multipurpose Senior Services Program. Other services provided include home set-up, mental health services, home delivered meals, food bank services, legal services, HUD assistance, public housing agency assistance, durable medical equipment, medical supplies, 24-hour caregiver, and STEP.

Areas Served by Lead Organizations

The lead organizations currently serve eleven counties:

·  HHCM serves northern California counties of Butte, Colusa, Glenn, Sacramento, Shasta, Sutter, Tehama, and Yuba.

·  ILR serves Contra Costa and Solano counties.

·  IAH and WCIL serve populations in Northwestern and Southern Los Angeles.

The CCT project team is working with organizations in San Francisco, Fresno, Alameda, Nevada, and Contra Costa Counties to develop new lead organizations.
