Co-Investigator and Lead trainer: Acknowledgement of Responsibilities

By signing below, as the Co-Investigator (co-I) and lead trainer, I acknowledge that I will be responsible for the following duties, as they apply to the project involving the use of and the use of BSL- containment practices:

1.  I am adequately trained in good microbiological techniques, including the safe handling and use of human agents and using BSL- containment practices.

2.  I will train the Principal Investigator (PI) and other lab personnel in the safe handling and use of and BSL- containment practices.

3.  I will instruct and train the PI and other laboratory personnel in: (i) the practices and techniques required to ensure safety, (ii) the risks associated with the agents, (iii) the procedures for dealing with accidents, and (iv) the reasons and provisions for any precautionary medical practices advised or requested.

4.  I will supervise the safety performance of the PI, and any other laboratory staff, to ensure that the required safety practices and techniques are employed during the use of .

5.  I will report any significant problems pertaining to the operation and implementation of containment practices and procedures to the BSO ().

6.  I will immediately report any serious accidents or potential exposures to the BSO, once the situation is stabilized.

7.  I will correct work errors and conditions that may result in the release of infectious materials.

8.  I will ensure the integrity of the physical containment (e.g., biological safety cabinets) and the biological containment (e.g., purity).

9.  I will make an initial determination of the required levels of physical and biological containment in accordance with the NIH Guidelines, and CDC BMBL.

10.  I will follow the VT IBC’s recommendations for the safe handling of .

11.  I will select appropriate microbiological practices and laboratory techniques to be used for the work.

12. I will make available to all laboratory staff the protocols that describe the potential biohazards and the precautions to be taken.
