Mood Survey

Below are a number of statements about your experience of moods. We would like you to consider your usual behaviour when you respond. Using the scale below, indicate the appropriate number to the right of each question and try to be as honest as you can.

1 = Strongly disagree

2 = Moderately disagree

3 = Somewhat disagree

4 = Somewhat agree

5 = Moderately agree

6 = Strongly agree

1.  I may change from happy to sad and back again several times in a single week.
2.  I usually feel quite cheerful.
3.  I’m frequently “down in the dumps”.
4.  I generally look at the sunny side of life.
5.  Compared to my friends, I’m less up and down in my mood states.

15. My friends often seem to feel I am unhappy.

Use the scale 1 to 99 contained in each question:

16. If 1 = very sad, 50 = neutral and 99 = very happy, how happy are you in general?
17. If 1 = hardly ever and 99 = extremely often, how often do your moods change?
18. If 1 = not at all and 99 = extremely strong, how strong are your moods?
