Athletic Physical Information Sheet
2016-2017 PIAA Sports Physical
The Spring Cove School District is sponsoring two free sports physicals for student-athletes who do not have a family physician or wish for the district to cover the costs for the annual sports physical that is mandated to participate in interscholastic athletics. This physical, when performed on or after June 1st, will cover all interscholastic sports for the 2015-2016 season. Physicals are performed at Dr. Heaton’s office, Heaton Family Practice (behind Nason Hospital, in Roaring Spring). Physicals can only be obtained by scheduling an appointment with Stacy Kuster, the school’s athletic trainer. She can be reached at (814) 934-9629. Physicals will be scheduled in the order the calls are received. Time slots fill up quickly and are on a first come first serve basics. These will be the only two school sponsored physical examination dates and the school will not have additional dates prior to the winter or spring sports season. If you wear corrective lens, please bring them with you. Dates for the school sponsored physical are:

PIAA Comprehensive Pre-Participation Physical Forms (CIPPE forms) Sections 1-5 must be completed and signed by the athlete and a parent prior to your scheduled appointment. CIPPE forms can be obtained from the main office at CHS, SCMS or via the PIAA website (www.PIAA.org). Due to time constraints, any athlete who shows up without a completed form or an appointment may be turned away. Please turn your completed physical form to the athletic secretary at CHS or to the coach on the first day of practice. You will not be able to participate in first day of practice if you have not turn in your physical. DO NOT TURN PHYSICALS INTO THE SCMS OFFICE.
The SCSD will be collecting the $25.00 registration fee for varsity and junior varsity athletes. Please make your check payable to Spring Cove Athletics and attached your check to your physical form. This fee covers all sports played for the current school year. If you have a financial hardship, please contact the athletic office.
