Anne Krupp Memorial Scholarship

The USTA/Midwest Tennis & Education Foundation Anne Krupp Memorial Scholarship has been created in partnership with Anne’s family and many of her friends.

Anne Krupp was a passionate tennis fan and advocate, working as a teaching professional, while putting much of her energy into working with Special Olympics and Adaptive and Wheelchair Tennis programs. Anne was a USTA/Midwest volunteer for 19 years serving in various capacities, and most recently served on the Adaptive Tennis Sub-Committee. Anne was a recipient of the USTA Eve Kraft Community Service Award in 2003.

The first $2,500 scholarship will be awarded beginning in the Spring of 2016. The scholarship will be presented to one or more USTA/Midwest Section tennis players of any age who need additional training or assistance with travel to participate in Special Olympics and/or Adaptive and/or Wheelchair Tennis, displays excellent sportsmanship on and off the court, and exhibits the true spirit of Anne.

Anne Krupp Memorial Scholarship Guidelines

Established in 1992, the USTA/Midwest Tennis & Education Foundation Inc. is the charitable and philanthropic entity of the USTA/Midwest Section. The Foundation is dedicated to supporting youth tennis programs, activities, and individuals throughout the USTA/Midwest Section. The Foundation is expanding its reach starting in 2017 to service adult populations which include Adaptive Tennis, Special Olympics, Wheelchair Tennis, and Wounded Warriors.

This scholarship application process is open to all interested parties. Applications are evaluated and approved by the Foundation Grant and Scholarship Review Committee. Individuals funded by the Foundation must reside within the USTA/Midwest Section that includes Illinois, Indiana, Michigan, Ohio, and Wisconsin, as well as designated counties in Kentucky and West Virginia.

Funding:

  • All factual information requested on the application and supporting documentation must be correctly stated.
  • The deadline to apply for this scholarship is March 1, and may be submitted via mail or email. The application must be postmarked if being mailed no later than March 1 or emailed to the Foundationn office on March 1.
  • This scholarship will be provided to one or more junior or adult players for an annualtotal of $2,500.
  • Guidelines will be followed unless exceptional circumstances warrant other consideration.
  • If applicant is not a US citizen, documentation of alien status must be submitted with the application.

  • Factors considered when evaluating applications: request for funds, annual improvement, length of time participating, financial need, and other resources available.
  • Applicantsmust demonstrate good sportsmanship.
  • It is the sole responsibility of the applicant (if he or she plays high school tennis) to contact their State High School Athletic Associations to determine that receipt of the scholarship from the Foundation will not adversely affect their eligibility to compete on any high school team or as an individual in a high school event or on a college/university team.

If a Foundation ScholarshipIs Awarded:

  • After approval by the Foundation Grant and Scholarship Committee, the Foundation office will contact the recipient, and inform them that if they are playing middle school or high school tennis, that they must contact their State High School Athletic Association regarding the parameters of the scholarship.
  • One-half of the approved scholarship will be forwarded to the recipient’s parents or guardians if a minor, and/or to the recipient if an adult.
  • The balance of the scholarship will be sent when the Foundation office receives a report, vouchers, and receipts for theentire awarded scholarship amountby a specified date provided to the recipient(s) upon notification of approval of funding which should include how the scholarship helped achieve the recipient’s goals.

Detailed Budget specific to grant requestfor the upcoming six month period(i.e., estimate all costs associated with program, travel, etc.)

  • At least one current letter of recommendation, including comment(s) on sportsmanship, from a tennis coach

  • If you wish to have financial need be a consideration, you must provide a copy of your most recent Federal Income Tax return (Form 1040, 1040A, or 1040EZ) (if you are a minor, submit parent’s); include W-2s and 1099s– For the security of applicants and their families, it is highly recommended social security numbers be blacked out or removed from all tax information submitted with the application.

AUTHORIZATION FOR USE OF NAME AND IMAGE

I, (printed name) ______do permit and authorize USTA/Midwest Tennis & Education Foundation, Inc. and its employees, agents, publishers, and personnel (“Authorized Parties”) who are acting on behalf of the Foundation to use my name and image for purposes related to the educational and philanthropic mission of the Foundation, including publicity, marketing, and promotion of the Foundation and its various programs without compensation to me. I understand my image may be copied and distributed by means of various media, whether now known or hereafter devised, including but not limited to photographs, video presentations, brochures, placement on the Foundation website, digital form, newspapers or other publications, or other print and electronic media. My image shall include photographs that I provide to the Foundation, as well as photographs that may be taken by the Foundation. I waive any right to inspect or approve the finished product or any material in which the Foundation may eventually use my name or image.

I understand that although the Foundation will endeavor to use my name and image in accordance with standards of good judgment, the Foundation cannot warrant or guarantee that any further dissemination of my name and image will be subject to the Foundation supervision or control. Accordingly, I release the Foundation and the Authorized Parties from any and all claims and liabilities related to dissemination of my name and image.

This Authorization for Use of Name and Image is effective the date set forth below and shall be binding on my heirs, successors, assigns, and legal representatives, and shall inure to the benefit of the legal representatives, successors, and assigns of the Foundation.

This Authorization for Use of Name and image shall be governed by the laws of the State of Indiana, may not be amended except in writing signed by both parties, and constitutes the entire agreement between me and the Foundation with respect to the subject matter contained herein.
