The aircraft documents (blue file) are complete and valid (no temporary certificates
acceptable).
The Aeroplane Flight Manual (AFM) including supplements, weight and balance data,
equipment list and other required operational documents are present and updated.
All relevant maintenance records are complete and up to date.
Corrective action has been taken and a corresponding report (if applicable) has been
submitted to FOCA, if the aircraft was subjected to objections at a previous FOCA
inspection.
The equipment required is installed and in a serviceable condition.

This form shall be submitted together with the certificate for passengerliability insurance (at least SDR 250’000) to Federal Office of Civil Aviation, Section Certification Flight Operations, 3003 Bern.
