EQUIPMENT SPECIFIC LOCKOUT TAGOUT PROCEDURES

Building/Equipment Location:West Building/Penthouse Mechanical Room

Equipment Description:HVAC Return Air

Purpose: This procedure establishes the minimum requirements for the lockout of energy isolating devices whenever maintenance or servicing is done on machines or equipment. It shall be used to ensure that the machine or equipment is stopped, isolated from all potentially hazardous energy sources and locked out before employees perform any servicing or maintenance where the unexpected energization or start-up of the machine or equipment or release of stored energy could cause injury.

Compliance With This Program: All employees are required to comply with the restrictions and limitations imposed upon them during the use of lockout. The authorized employees are required to perform the lockout in accordance with this procedure. All employees, upon observing a machine or piece of equipment, which is locked out to perform servicing or maintenance, shall not attempt to start, energize or use that machine or equipment.

SPECIFIC INSTRUCTION FOR LOCKOUT

  1. Survey and identify energy sources and their associated hazards and obtain equipment.
  1. Notify all affected employees that a lockout tagout system is going to be utilized and the reason why the equipment is being locked out.
  1. If the equipment is in the “on” or “powered” position, shut it down by the normal stopping procedure.
  1. Isolate the equipment from its energy source by placing your lock and tag on the electrical disconnect shown below.

HOW TO VERIFY LOCKOUT BEFORE PERFORMING MAINTENANCE

  1. After ensuring that no other personnel are exposed, attempt to turn on the equipment using the disconnect shown below to ensure that the lockout was effective.
  1. After ensuring that lockout was effective, return the equipment to the “off” position. Equipment is now locked out and ready to be serviced.
INSTRUCTIONS FOR RETURNING MACHINE TO SERVICE
  1. Check the area around the equipment to ensure that no one is exposed.
  1. Verify that all tools have been removed from the equipment and all guards have been reinstalled.
  1. Notify affected employees of the impending re-energization of the equipment.
  1. Remove the lockout and tagout devices. Removal of the lockout and tagout devices is limited to the authorized employee who applied the device. See abandoned lock procedure if the lockout and tagout devices were inadvertently left on the equipment.
  1. Energize the equipment.
