West Jessamine High School Attendance/Prom Policy Appeal Form

In October, 2010, the West Jessamine High School Site Based Council adopted the following policy in order to promote the importance of school attendance:

Any student who has 15 or more combined absences will not be permitted to attend West Jessamine High School prom. (Students will have the option to file an appeal to explain special circumstances.)

Completion of this form serves at the initial step for a student to file an appeal of the policy stating why the large number of absences was due to circumstances outside of the control of the student. A committee of school representatives will review each form individually and make one of three rulings:

  1. The absences of the student are primarily due to documented circumstances or illness outside of the students control warranting the policy being waived and the student permitted to attend prom.
  2. The absences of the student are not found to be primarily due to documented circumstances or illness outside of the student’s control, and the student is not permitted to attend to prom.
  3. Further information is required and the student must appear before the appeals committee before a decision will be made. The committee at this time may put in place an attendance contract regarding attendance up to, and following, prom.
