Parental Consent Form

For your child’s safety and the peace of mind of all the parents, a few rules are set forth and must be agreed upon by all parents and students (minors):

The possession or consumption of any alcoholic beverage is unlawful when consumed by minors. If at any time during the course of service any student (minor) is found in the possession of any illegal substance, including alcohol or drugs. The chauffeur will immediately terminate the service and all vehicle occupants (minors) will be returned to the consenting parent. The Chauffeur shall also have the right to examine all bags and/or parcels to be placed in the vehicle to determine if they contain any illegal substances.

The moon roof will not be operational. The moon roof will be closed at all times. No one is allowed to put any body parts outside the side window and there will be no smoking allowed inside the vehicle.

Students (minors) agree to return vehicle in the same condition as when received at start of service, and to use said vehicle at all times in a proper, prudent and reasonable manner. Parent further agrees that he or she will be liable for any items damaged or defacement of the vehicle at actual costs to repair.

I have carefully read this agreement and fully understand its contents. I will be responsible for my minor child, as well as all other minors riding in the vehicle. I am fully aware that upon any infractions of this agreement all monies shall be forfeited. I am aware that this is a release of liability and a contract between me and Alliance Limousine Service, Inc.
